ARTICLE 12 -MEDICAL BENEFITS/INSURANCE <br /> 1. EMPLOYEE AND DEPENDENT MEDICAL: <br /> A. The City agrees to offer the City's self-insured and Kaiser Permanente plan. The employee will <br /> make a 10%contribution to the cost of medical coverage. <br /> The City also agrees to offer the City's new Consumer Health Driven Plan. The employee will <br /> make a 5%contribution to the cost of medical coverage. The City will make total contributions to <br /> an HRA in the amount of$1,500 for an individual or$3,000 for a family. These contributions will <br /> be made monthly in 12 equal amounts. Changes in coverage,will have an adjusted HRA <br /> contribution,effective the first month following the change in coverage. Employees and their <br /> covered spouse that take a whole health exam at the VERA Clinic will receive an additional VEBA <br /> contribution in the amount of$200 each. <br /> As an option,the City also agrees to allow employees and their dependents to choose the LEOFF <br /> Health and Welfare Trust Plan B.The employee will make a 10%contribution to the cost of <br /> medical coverage. The City will make contributions to an HRA in the amount of$166.66/month for <br /> employee only coverage or$333.33/month for family coverage if the employee enrolls in the <br /> LEOFF Trust Plan. <br /> B. The employee has the choice of plans,but the plan must be the same for all family members. <br /> 2. DENTAL COVERAGE: The City will pay 100%of the monthly premium for the basic dental <br /> insurance plan offered through Washington Dental Service or Willamette Dental Group. Said <br /> dental insurance will be provided for all bargaining unit members and their dependents,with an <br /> annual maximum of$2,000. <br /> 3. In lieu of the City's historical practice of making a modest contribution toward the purchase of a <br /> disability income insurance plan for employees,the City will make a monthly contribution to the <br /> Premium Reimbursement Plan of the Washington State Council of Fire Fighters Employee Benefit <br /> Trust(the"Plan")on behalf of each employee. The monthly contribution will be the equivalent of <br /> one hour's pay for a First-Class Fire Fighter at the then-applicable rate. <br /> By making such contributions,the City does not make any representations concerning the tax <br /> treatment of City contributions or of payments made by the Plan and relies on the Plan's <br /> representation that the Plan will properly withhold and report all such payments in accordance with <br /> applicable law,including for payments made to domestic partners. Further,the City does not agree <br /> to make any further contributions to the Plan for any other liability or claim beyond the monthly <br /> contributions described above. In the event of any claim by the Plan or any other entity or <br /> individual against the City,other than concerning the above-described monthly contributions,the <br /> Union agrees to indemnify and hold the City harmless from and against any and all liability related <br /> thereto, including for the City's attorney's fees and associated costs of defense. <br /> 4. WAPFML Language <br /> The City will provide Washington Paid Family Medical Leave for eligible employees, in <br /> accordance with RCW 50A.04, at no cost to the employee for year.Effective January 1,2021, <br /> employees may be responsible for paying the employee's share of the WAPFML premium. The <br /> City agrees to notify the union of this deduction 90 days prior to the start of the deduction and <br /> negotiate any impacts of such deduction. <br /> 15 <br />
