O„..„
0 1
PU®L/C WORKS
November 6, 2013
Nikki Oku
1902 Lombard Avenue
Everett, WA 98201
Re: Backwater Valve Installation at 1902 Lombard Avenue
Dear Ms. Oku:
The City's contractor, Steve's Plumbing, inspected your plumbing and drainage on September
19, 2013 to determine if a backwater valve is appropriate for your property. Their findings
indicate that installing a backwater valve would be beneficial. Please review the enclosed
report.
Previously, you signed the Backwater Valve Authorization Form authorizing the city to install
a backwater valve at your residence. Based on the inspection report, we are proceeding with
the installation. The City's contractor, Tellus LLC, will be contacting you in November to
schedule the work. This will require some excavation on your property.
The City will provide you on-site training on the operation and maintenance of the backwater
valve. Maintenance of the valve and the plumbing, sewer, and drainage on your property are
your responsibility. The inspection identified additional site drainage and plumbing
deficiencies in your private drainage and plumbing system. These include:
• Apparent side sewer break
• Additional work could be completed to seal exterior walls from groundwater intrusion
These deficiencies are your responsibility to address, as you desire. The City is not
responsible for future sewer or storm damages that may result from improper maintenance
of, or deficiencies within, your private plumbing and drainage system.
If you have any questions about this process, please contact Grant Moen at 425=257-8947 or
gmoen@everettwa.gov, or Brian Doolan at 425-257-8828 or bdoolan@everettwa.gov.
Thank you for your interest in our backwater valve program.
rel
Mark Sad .E.
Maintenance Superintendent /
Everett Public Works
CITY OF EVERETT• 3200 Cedar Street• Everett, WA 98201 • (425)257-8800 • Fax (425)257-8882
