Project: Water Main Replacement"V" <br /> Change Order No. <br /> Change Order Effective Date: June 7, 2022 <br /> Exhibit A—Description of Changed Work <br /> This Change Order No. 1 modifies one (1) bid item and adds three (3) new items to the Contract, <br /> as described below. <br /> MODIFIED BID ITEM <br /> Bid Item #41 —Crushed Surfacing Top Course. Modify bid item#41 entitled, "Crushed <br /> Surfacing Top Course" at the bid item unit price of$41.85/Ton, by adding 75 tons to the <br /> Contract, resulting in a Contract increase of$3,138.75. <br /> ADDED ITEMS <br /> New Item # 100—Cement Concrete Sidewalk Restoration Equitable Adjustment. Add new <br /> item entitled, "Cement Concrete Sidewalk Restoration Equitable Adjustment" at the agreed to <br /> Unit Price of$92.96/SY, by adding 84.4 square yards to the Contract, resulting in a Contract <br /> increase of$7,845.82 to the Contract price. <br /> New Item # 101 —Provide and Install Telemetry Wire. Add new item entitled, "Provide and <br /> Install Telemetry Wire" at the agreed to Lump Sum price of$6,089.00, resulting in a Contract <br /> increase of$6,976.00. <br /> New Item # 102 —Provide and Install Field Lock Gaskets on 8-Inch and 12-Inch Ductile <br /> Water Main Pipe. Add new item entitled, "Provide and Install Field Lock Gaskets on 8-Inch <br /> and 12-Inch Water Main Pipe" at the agreed to Lump Sum price of$5,513.00, resulting in a <br /> Contract increase of$5,513.00. <br /> JUSTIFICATION <br /> Bid Item #41 —Crushed Surfacing Top Course. This bid Item exceeded plan quantity and is <br /> being modified to account for additional quantities of this bid item. <br /> New Item # 100—Cement Concrete Sidewalk Restoration,Equitable Adjustment. Bid Item <br /> #46-Cement Concrete Sidewalk Restoration only required 84.4 SY of quantities, which is 38% <br /> of Plan Quantity. In accordance with WSDOT Standard Specifications 1-04.6,this bid item has <br /> been renegotiated to $140.00/SY, replacing Bid Item# 46 Unit Price of$57.00/SY, so that 140- <br /> 57=83.00/SY plus markup=92.96/SY is the difference between the bid item price and the <br /> renegotiated item price. <br /> New Item # 101 —Provide and Install Telemetry Wire. City of Everett Traffic Division's <br /> Telemetry wire was mismarked by the locater on Pine Street at Hewitt. The contractor <br /> accidentally hit the conduit, causing damage to the 25-pair wire. This new item is being created <br /> to account for costs to purchase and install wire to replace the damaged wire inside the conduit. <br />
