1 <br /> CITY OF EVERETT SPECIAL PROVISIONS <br /> 1. A complete statement of the origin, composition, and manufacture of any or <br /> ' all materials to be used, <br /> 2. Samples of these materials for quality and fitness tests, <br /> 3. A progress schedule, in a form the City requires, showing the order of and <br /> time required for the various phases of the Work, <br /> 4. A breakdown of costs assigned to any bid item, <br /> 5. Attendance at a conference with the Engineer or representatives of the <br /> 1 Engineer, <br /> 6. Obtain, and furnish a copy of, a business license to do business in the City of <br /> Everett. <br /> 7. A copy of State of Washington Contractor's Registration, or <br /> 8. Any other information or action taken that is deemed necessary to ensure that <br /> the Bidder is the lowest responsible bidder. <br /> 1-03 AWARD AND EXECUTION OF CONTRACT <br /> 1-03.1 Consideration of Bids <br /> Revise the first paragraph to read: <br /> After opening and reading Proposals, the City will check them for correctness of <br /> extensions of the prices per unit and the total price. If a discrepancy exists between the <br /> price per unit and the extended amount of any bid item, the price per unit will control. If <br /> a minimum bid amount has been established for any item and the Bidder's unit or lump <br /> sum price is less than the minimum specified amount, the City will unilaterally revise the <br /> unit or lump sum price to the minimum specified amount and recalculate the extension. <br /> The total of extensions, corrected where necessary, including sales taxes where <br /> applicable and such additives and/or alternates as selected by the City, will be used by <br /> the City for Award purposes and to fix the awarded Contract Sum and the amount of the <br /> Contract Bond(s). <br /> Revise the third and fourth paragraphs of 1-03.1 to read as follows: <br /> Within 5 days after the opening of Proposals, a Bidder who wishes to claim error shall <br /> submit a notarized affidavit signed by the Bidder, accompanied by original work sheets <br /> used in the preparation of the Proposal, requesting relief from the responsibilities of <br /> Award. <br /> ' The affidavit shall describe the specific error(s) and certify that the work sheets are the <br /> originals used in the preparation of the Proposal. The Engineer will review the certified <br /> work sheets to determine the validity of the claimed error and make recommendation to <br /> the City. If the City concurs in the claim of error, the Bidder will be relieved of <br /> responsibility, and the bid deposit of the Bidder will be returned. Thereafter, at the <br /> discretion of the City, all Bids may be rejected or Award made to next lowest and <br /> responsive Bidder. <br /> Supplement 1-03.1 by adding the following: <br /> 1-03.1(2) Preference for Resident Contractors <br /> (******) <br /> In accordance with RCW 39.04.380, if a Bid is received from a nonresident contractor <br /> from a state that provides a percentage bidding preference and does not have an office <br /> located in Washington, then a comparable percentage disadvantage will be applied to <br /> the Bid of that nonresident contractor. <br /> 1-03.2 Award of Contract <br /> Revise 1-03.2 to read as follows: <br /> Water Main Replacement"W" Division 1 —GENERAL REQUIREMENTS April,2022 <br /> WO No.—UP3766 SP—49 <br /> I <br />
