SCHEDULE A -1 <br />Liability Claims Management Services <br />Terms and Conditions <br />I. GENERAL SERVICES <br />(a) Subject to the Customer's direction, CorVel shall supervise and administer the <br />Liability Claims Adjustment Program for the Customer and shall act as the <br />Customer's representative in connection with the investigation, adjustment, <br />processing, supervision and resolution of liability claims by third parties against <br />the Customer. <br />(b) CorVel shall undertake periodic program and administrative reviews on at least a <br />quarterly basis to Customer as to any CorVel recommendations for changes or <br />improvements in the administration of the third party claims program. <br />(c) CorVel shall invoice Customer a flat fee per claim and/or separate suffix hereunder <br />this Exhibit A-1. Such pricing is not occurrence based under the Services provided <br />by CorVel. <br />(d) CorVel shall manage the following types of claims: <br />o <br />o <br />o <br />o <br />General Liability <br />Personal Injury <br />Professional Liability <br />Product Liability <br />Auto Liability <br />Auto Physical Damage <br />Other (Specify): <br />• <br />• <br />• <br />II. INVESTIGATIVE SERVICES <br />(a) CorVel shall provide investigative services to include, but not limited to: <br />(i) <br />Receipt and examination of all reports of accidents, incidents, and claims <br />cases which are or may be the subject of such liability claims; <br />(ii) Investigation of such accidents, incidents, claims or cases where <br />examination warrants such investigation to include on -site investigation, <br />photographs, statements of clients, claimants and witnesses, evaluation <br />and determination of losses, and other such investigative services <br />necessary to determine liability and losses, but not to include extraordinary <br />investigative services as set forth in Section b(iii) below; <br />(iii) Timely and periodic reporting to the Customer of the in -progress <br />investigation efforts and the results of the investigations, as well as <br />offering recommendations to the Customer of extraordinary investigative <br />services, if required. <br />City of Everett, WA — CorVel EC Amendment 3 7-25-2022 <br />
