1 <br /> City of Everett 00 21 13 - 2 <br /> Water Filter Plant Emergency Generator Replacement WO#3587 1 <br /> 3. State Requirements. If applicable: ' <br /> a. Have Industrial Insurance (workers' compensation) coverage for the bidder's <br /> employees working in Washington, as required in Title 51 RCW; <br /> b. Have a Washington Employment Security Department number, as required in111 <br /> Title 50 ROW; and <br /> c. Have a Washington Department of Revenue state excise tax registration <br /> number, as required in Title 82 RCW. <br /> 4. Disqualification. Not be disqualified from bidding on any public works contract <br /> under RCW 39.06.010 or 39.12.065(3). <br /> 5. Prevailing Wage Training. Unless Bidder has completed three or more public works <br /> projects and had a valid business license for three or more years, Bidder must have <br /> received Department of Labor and Industries training on the requirements related to <br /> public works and prevailing wage under RCW 39.12 and RCW 39.04. <br /> 5. Certification of Wage Compliance. Within the three-year period immediately <br /> preceding the date of the bid solicitation, not have been determined by a final and <br /> binding citation and notice of assessment issued by the department of labor and111 <br /> industries or through a civil judgment entered by a court of limited or general <br /> jurisdiction to have willfully violated, as defined in RCW 49.48.082, any provision of <br /> chapter 49.46, 49.48, or 49.52 RCW. <br /> 6. Apprentices. If the Project is subject to the apprenticeship utilization requirements <br /> in RCW 39.04.320, not have been found out of compliance by the Washington state <br /> apprenticeship and training council for working apprentices out of ratio, without <br /> appropriate supervision, or outside their approved work processes as outlined in <br /> their standards of apprenticeship under chapter 49.04 RCW for the one-year period <br /> immediately preceding the date of the bid solicitation. <br /> 1-02.1(2) Supplemental Bidder Responsibility Criteria <br /> If this Project is subject to supplemental bidder responsibility criteria, then such criteria <br /> will be contained in SECTION 00 2213, SUPPLEMENTAL INSTRUCTIONS TO <br /> BIDDERS. If there is no SECTION 00 2213, SUPPLEMENTAL INSTRUCTIONS TO <br /> BIDDERS in the bid package, then the Project is not subject to supplemental bidder <br /> responsibility criteria. <br /> 1-02.2 Plans and Specifications <br /> Information as to where bid documents can be obtained or reviewed will be found in the <br /> Call for Bids (Advertisement for Bids) for the Work. <br /> 1-02.3 Not Used <br /> 1-02.4 Examination of Plans, Specifications, and Site of Work <br /> 1-02.4(1) General I <br /> The Bidder shall carefully examine the Contract Documents. Submittal of a Bid shall be <br /> conclusive evidence that the Bidder has made these examinations and understands all <br /> requirements for the performance of the completed Work. The Bidder further warrants, <br /> agrees, and acknowledges by submitting a Bid that it: <br /> 1. Has taken steps reasonably necessary to ascertain the nature and location <br /> of the Work, including without limitation the actual physical conditions of and I <br /> at the location, surface and subsurface conditions, and conditions ordinarily <br /> to be encountered and generally recognized as inherent in the Work; <br /> 1 <br /> 00 21 13 - 2 INSTRUCTIONS TO BIDDERS <br /> 1 <br />
