The Agency shall comply with all applicable terms and conditions of the <br />Business Associate Agreement between the County and the Agency <br />referenced on the Contract Face Page. <br /> <br />IV. PROJECT MANAGEMENT <br /> <br />Recognizing that the body of knowledge regarding COVID-19 is rapidly evolving, <br />the Agency shall take all reasonable measures, based on the existing state-of-the- <br />art knowledge related to the transmission of COVID-19 as made available by the <br />U.S. Centers for Disease Control and Prevention, the Washington State <br />Department of Health, and the Snohomish Health District, to prevent the spread of <br />COVID-19. <br /> <br />A. Location and Hours of Service <br /> <br />Services provided under this Agreement shall be available in Snohomish <br />County for Snohomish County residents as specified in Exhibit B. <br /> <br />B. Individual Grievances <br /> <br />The Agency shall have policies and procedures in place for individual <br />grievances in the case of denial or termination of services or failure to act upon <br />a request for services with reasonable promptness. <br /> <br />C. Toll-Free Telephone Services <br /> <br />The Agency shall maintain a telephone system that provides for toll-free calls <br />for all residents of Snohomish County using the services described herein. <br /> <br />D. Change in Personnel <br /> <br />The Agency shall notify the County within ten (10) days of change in personnel <br />which may affect the faithful execution of this Agreement. <br /> <br />E. Subcontracting <br /> <br />The Agency is prohibited from subcontracting any funding and/or services <br />contained within the Agreement unless otherwise negotiated with the County. <br />All subcontracting arrangements require prior written approval from the County. <br /> <br />F. Emergency Procedures <br /> <br />The Agency shall have a plan for serving individuals during periods when <br />normal services may be disrupted. Disruptions to normal services may include, <br />but are not limited to, earthquakes, floods, snowstorms, other natural disasters,
