1.0 PURPOSE <br />The purpose of these inspections and tests is to provide assurance that custom <br />or non -certified equipment meets the requirements of the appropriate codes, <br />safety orders and industry standards. These inspections and tests are normally <br />required by the local building inspection authority having jurisdiction (AHJ) when <br />equipment has not been certified by a Nationally Recognized Testing Laboratory <br />(NRTL) or other recognized testing agency. These inspections address only the <br />electrical systems on the equipment listed in Section 5.0 of this report. <br />2.0 SUMMARY <br />This project was initiated at the request of Mr. Brian Hainey with Electroimpact. <br />Inspection, testing and labeling of the equipment referenced in Section 5.0 was <br />performed at the Boeing 40-27 Building in Everett, WA by eti Conformity Services <br />Compliance Engineer Donald Preiss on November 15, 2022. No discrepancies <br />were noted. <br />The equipment was previously approved with eti labels 2025743 and 2025798. <br />Scanners were added and the equipment retested and labeled. <br />No electrical discrepancies were observed during the inspection and testing of <br />the equipment. The scanners and one existing E Stop were tested. eti Conformity <br />Services considers this equipment to be in compliance with the electrical codes <br />and standards listed in Section 4.0, pursuant to the conditions of acceptance <br />described in Section 3.0. An eti Conformity Services evaluation label has been <br />affixed to the equipment. Note that final approval is under the jurisdiction of the <br />City of Everett. <br />3.0 GENERAL CONDITIONS OF ACCEPTANCE <br />In the event of modifications resulting in a change in the materials, manufacturing <br />methods, loading, or environment that would affect the use of the accepted <br />equipment under the provisions of the noted electrical standards, this acceptance <br />will be considered automatically canceled. The applicant will be required to <br />request re-examination of this equipment to determine acceptability of the <br />modifications. <br />By acceptance of the equipment referenced in Section 5.0, eti Conformity <br />Services does not assume or discharge the responsibility of the equipment <br />manufacturer, installer, or other relevant parties. Equipment evaluation is based <br />upon adherence to sound engineering practices, and upon compliance with the <br />specific sections quoted from the electrical standards referenced in Section 4.0 of <br />this report. <br />Reference No.: 1099539 Page 1 <br />
