With the funding requested, we would solicit and evaluate proposals from <br />providers of advanced health screening services to bring mobile cardiac <br />screening to a location convenient for our officers. <br />b. Describe the program's outcomes to be measured and your plan to meet them. <br />The primary outcome is participation in the screening program and our goal is <br />100% participation by all of our officers. Participating in the screening will help <br />officers understand their risk level, as well as actions they can take to reduce risk <br />factors and improve their overall health and well-being. This in turn contributes <br />to a healthier work force overall in the long run. <br />Outcomes we will measure include: <br />• Number of officers participating in the program <br />• Officer satisfaction with the screening process, which we'll gather <br />through an employee survey <br />• Reduction in preclinical coronary artery disease, which will be assessed <br />through the screening process <br />Deputy Chiefieraud Irving will be responsible for organizing the screening <br />opportunity and promoting it internally to maximize participation. He will also <br />collect data on program participation and conduct a follow-up employee survey <br />to gauge satisfaction with the program. <br />Everett PD intends to seek additional funds in the future to be able to offer this <br />wellness assessment every five years. This will allow for better tracking of health <br />outcomes over time. <br />c. Identify other organizations, partners,, or funders participating in the project and <br />their roles. <br />This project will primarily involve the Everett Police Department Everett PD <br />intends to coordinate with our Human Resources Department to engage the <br />City's benefit broker to procure and administer the health screening program to <br />our officers. We plan to enter into an agreement with an outside provider to <br />conduct the screening service. <br />Page 4 of 5 <br />
