a) Claims for reimbursement must be submitted on approved forms within five(5) <br /> working days of purchase date. Reimbursement forms will be supplied by the <br /> employee's department. <br /> b)Such claim for reimbursement must be signed by the claimant and the claimant's <br /> supervisor and must include the receipt for the earned meal. <br /> Section 15: Rest Periods <br /> Employees shall receive a fifteen(15)minute paid rest period for each four(4)hours,or major portion <br /> thereof, of their working time. With their supervisor's approval,employees may be authorized to take <br /> their breaks on an intermittent basis but may not exceed fifteen(15)minutes. An employee who does not <br /> receive a rest period will be compensated at the appropriate rate of pay for each missed rest period. Rest <br /> periods will generally be taken at job site. <br /> Section 16: Shift Differential <br /> Employees working a regularly assigned shift beginning at 2:00 p.m.and before 2:00 a.m. shall receive an <br /> additional 5%pay per hour above the individual's current base salary. <br /> Section 17: Union Logos <br /> Employees shall be allowed to wear the Local, State Council or International logo of the American <br /> Federation of State, County and Municipal Employees;or County, State or International logo of the AFL- <br /> CIO on their lapel or work attire so long as it is consistent with department policies. <br /> Section 18: Safety Boots and Shoes <br /> Safety boots or shoes will be reimbursed for employee positions on the negotiated approved list <br /> (Addendum B)up to an annual maximum amount of$200.00. Safety boots and shoes must be worn in <br /> accordance with department policy and procedures.Any changes to the approved list shall be addressed in <br /> labor management. <br /> 23 <br />
