Section 5: Dental. <br /> The City will pay the premium for the dental plan 12,offered through the trustees of the Washington <br /> State Council of County and City Employees for the employee and their eligible dependents. <br /> Section 6: Vision. <br /> The City will pay the premium for the vision plan offered through the trustees of the Washington State <br /> Council of County and City Employees for the employee and their eligible dependents. <br /> Section 7: Enrollment. <br /> In order to participate,an employee must sign authorization cards to be kept on file by the City. If the <br /> employee does not wish to participate in the program,then they must sign a waiver card for their file. <br /> Section 8: Disability Insurance. Employees covered under this agreement shall participate in a <br /> disability insurance program through the insurance company of the Union's choice. Premiums for this <br /> coverage will be the responsibility of the employee. <br /> Section 9: Review. <br /> The City will provide reports on the City's self-insured plan upon request which will include usage trends <br /> and fund balance. <br /> Section 10:(Washington Paid Family Medical Leave(WAPFML) <br /> The City will provide Washington Paid Family Medical Leave for eligible employees,in accordance with <br /> RCW 50A.04,at no cost to the employee for year 2023. Effective January 1,2024,employees may be • <br /> responsible for paying the employee's share of the WAPFML premium. The City agrees to notify the <br /> union of this deduction 90 days prior to the start of the deduction and negotiate any impacts of such <br /> deduction. <br /> Following the finalization of regulations implementing RCW 50A.04,either party may reopen this <br /> agreement for the purposes of bargaining over issues related to the interrelations between leaves available <br /> under this agreement and benefits provided by statute. <br /> 30 <br />
