ARTICLE 12 - INSURANCE BENEFITS <br /> Section 1: Eligibility. <br /> a. New hire employees shall not receive any City paid insurance benefits until the first day <br /> of the 3rd month of employment(employee starts work in April,insurance starts June <br /> 1st). <br /> b. Regular part-time employees who work 22 hours or more per week shall be eligible for <br /> benefits under Article 12 on a pro-rata basis. <br /> c. Day laborers(IV)will be eligible to participate in medical insurance.Returning Day <br /> Laborer IV's shall receive medical insurance on the first day of the following month of <br /> rehire <br /> d. Circulation Assistant I/Shelvers are exempt from Article 12 unless required by law. <br /> Section 2: Medical Insurance. <br /> The City agrees to offer medical coverage for eligible employees and their legal dependents. Employees <br /> and their dependents shall have the option of enrolling in the Kaiser,-HMO Plan or one of the City's self- <br /> insured medical plans. Employees that choose the Kaiser HMO Plan or the City's traditional PPO Plan, <br /> shall pay ten percent of the monthly premium. Employees that choose the City's Consumer Driven <br /> Healthcare Plan(CDHP),shall pay 0%of the monthly premium share in 2023 and 5%of the monthly <br /> premium in 2024. . <br /> Employees that enroll in the CDHP will receive a City paid contribution to an HRA/VEBA in the amount <br /> of$1,200 for individual for$2,400 for family coverage. Changes due to a qualifying event will be pro- <br /> rated for the remainder of the year. <br /> Effective January 1,2024,Employees enrolled in the CDHP will receive quarterly contributions in their <br /> HRA/VEBA(paid at beginning of each quarter)in the amount of$300 for individual coverage and$600 <br /> for family coverage. Changes due to a qualifying event will be pro-rated for the remainder of the year. <br /> Annual Whole Health Evaluation: <br /> Employees enrolled in the CDHP will receive a HRA/VEBA contribution in the amount of$200 for <br /> individual and$200 for spouse or domestic partner upon completion of an Annual Whole Health <br /> Evaluation. <br /> Employees enrolled in the traditional PPO Plan will receive a HRA/VEBA contribution in the amount of <br /> $100 upon completion of an Annual Whole Health Evaluation. <br /> The benefits under the City's self-insured medical insurance program are described in Appendix 2,and <br /> such benefits shall not be reduced without agreement of the parties. <br /> Section 3: Life Insurance. <br /> The City will provide for the employee$1,000 life insurance per$1,000 of annual salary,rounded to the <br /> next highest$1,000 at the City's expense for the duration of this agreement. <br /> Section 4: AD&D. <br /> The City will provide for the employee Accidental Death and Dismemberment coverage in an amount <br /> equal to that specified in Section 3 hereinabove,at the City's expense for the duration of this agreement. <br /> 29 <br />
