EXHIBIT D <br />Setting up your eligibility files <br />To offer Carrot, you'll need to provide regularly <br />updated eligibility files. Here's how: <br />File name <br />Company_Eligibility YYYY-MM-DD,csv <br />"Company" is your company's name <br />"YYYY-MM-DD" Is the date of the file's creation <br />Fields and formatting <br />First name (,-equired) The employee's first name. <br />Last name (required) — The employee's last name. <br />Email (optional) — The email of the employee, if available. <br />Employee ID (required) — The employee ID or unique identifier. <br />Note: This should not be SSN. <br />Eligibility date ;required) The date when an employee is eligible for the Carrot benefit, generally <br />the date of hire.
