I <br /> 1 1. The approved "Statement of Intent to Pay Prevailing Wages" State L&I's form <br /> I 2 number F700-029-000. The Contracting Agency will make no payment under this <br /> 3 Contract until this statement has been approved by State L&I and reviewed by <br /> 4 the Engineer. <br /> I 5 <br /> 6 2. The approved "Affidavit of Prevailing Wages Paid", State L&I's form number <br /> 7 F700-007-000. The Contracting Agency will not grant Completion until all <br /> I 8 approved Affidavit of Wages paid for the Contractor and all Subcontractors have <br /> 9 been received by the Engineer. The Contracting Agency will not release to the <br /> 10 Contractor any funds retained under RCW 60.28.011 until "Affidavit of Prevailing <br /> 11 Wages Paid" forms have been approved by State L&I and all of the approved <br /> 12 forms have been submitted to the Engineer for every firm that worked on the <br /> 13 Contract. <br /> 14 <br /> I 15 The Contractor is responsible for requesting these forms from State L&I and for paying <br /> 16 any fees required by State L&I. <br /> 17 <br /> I 18 Certified Payrolls <br /> 19 Certified payrolls are required to be submitted by the Contractor for themselves, all <br /> 20 Subcontractors and all lower tier subcontractors. The payrolls shall be submitted weekly <br /> I 21 on all Federal-aid projects and no less than monthly on State funded projects. <br /> 22 <br /> 23 Penalties for Noncompliance <br /> I24 The Contractor is advised, if these payrolls are not supplied within the prescribed <br /> 25 deadlines, any or all payments may be withheld until compliance is achieved. In addition, <br /> 26 failure to provide these payrolls may result in other sanctions as provided by State laws <br /> 27 (RCW 39.12.050) and/or Federal regulations (29 CFR 5.12). <br /> I 28 <br /> 29 1-07.11.GR1 <br /> 30 Requirements for Nondiscrimination <br /> I 31 <br /> 32 1-07.11.INST1.GR1 <br /> 33 Section 1-07.11 is supplemented with the following: <br /> I 34 <br /> 35 1-07.11.OPT1.GR1 <br /> 36 (September 3, 2019) <br /> 37 Requirement for Affirmative Action to Ensure Equal Employment Opportunity (Executive <br /> 38 Order 11246) <br /> 39 <br /> 40 1.
