<br />  <br />Exhibit "A" <br />Scope of Services, Time of Performance, Budget, <br />Payment, Notices, and Representatives <br />A. Activities/Project Description <br /> <br />The Subrecipient will be responsible for administering the Minor Home Repair program in a manner <br />satisfactory to the City and consistent with any standards required as a condition of providing these <br />funds. Such program will include the following activities eligible under the Community Development <br />Block Grant Program: <br /> <br />Program Delivery <br /> <br />Activity: <br /> <br />The Minor Home Repair program will serve 76unduplicated households by providing necessary <br />health, safety, and accessibility related repairs to vulnerable adults that they cannot perform or <br />afford, based on the General Administration <br /> <br />General Administration <br />[Description of general administrative services to be performed in support of activities noted above] <br /> <br />Houses and Mobile Homes on private property: <br />A) Services provided by the Home Repair Program may include plumbing repairs, furnace <br />repairs or replacement, roof patching, window replacement, carpentry, grab bars, door locks, <br />smoke alarms, accessibility modifications and other health and safety related home repairs. <br />The Home Repair program allows a homeowner up to $5,000 for minor home repairs in a <br />one-year contract period. <br />B) Minor Home Repair projects will be limited to $5,000.00 per year of actual dollars spent (labor <br />and materials) on single-family homes on lots/property owned by the homeowner. Once this <br />dollar amount has been reached then the client will be referred to the City of Everett’s <br />Community Housing Improvement Program (CHIP). This amount may be increased by the <br />City of Everett after CHIP reviews the property to assess if the property/unit and client are <br />eligible for CHIP funding. For clients who do not qualify for CHIP, with City of Everett pre- <br />approval, additional work may be done by the Home Repair Program up to $3,000.00 per <br />year, for a total of no more than $8,000.00 total project funds per year. This amount may be <br />increased with review and approval by the City of Everett after review of the safety needs of <br />the home. <br /> <br />Mobile Homes on leased/rented land: <br />Services provided by the Home Repair Program may include plumbing repairs, furnace <br />repairs or replacement, roof patching, window replacement, carpentry, grab bars, door locks, <br />smoke alarms, accessibility modifications and other health and safety related home repairs. <br />The Home Repair program allows a homeowner up to $5,000 for minor home repairs in a <br />DocuSign Envelope ID: CD12F8A2-AA45-4204-8F1C-E2B1DD0D2785
