<br />Exhibit "A" <br />Scope of Services, Time of Performance, Budget, <br />Payment, Notices, and Representatives <br /> <br />I. Scope of Service <br /> <br />A. Activities/Project Description <br /> <br />The Subrecipient will be responsible for administering the Transitions Behavioral Health Coordinator <br />program in a manner satisfactory to the City and consistent with any standards required as a <br />condition of providing these funds. Such program will include the following activities eligible under <br />the Community Development Block Grant Program: <br /> <br />Program Delivery <br /> <br />Activity: <br /> <br />The Transitions Behavioral Health Coordinator will serve fifteen (15) Persons by providing behavioral <br />health crisis management support and referrals to residents of Cocoon House’s Transitions program. <br /> <br />General Administration <br />[Description of general administrative services to be performed in support of activities noted above] <br /> <br />Cocoon House will provide all necessary administrative support to this program including agency <br />leadership, human resources, and sound financial practices. Cocoon House maintains agency <br />policies and procedures which include a non-discrimination plan and grievance procedure. The <br />agency maintains an accounting, financial reporting, and financial management and operations <br />system in order to comply with Generally Accepted Accounting Principles (GAAP). Cocoon House <br />also retains all necessary licenses and certifications including city and state business licenses. <br />Program staff receive regular training, supervision, and professional development opportunities to <br />ensure participants receive quality care and services. <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />
