Exhibit "A" <br />Scope of Services, Time of Performance, Budget, <br />Payment, Notices, and Representatives <br />l. Scope of Service <br />A. Activities/Proiect Description <br />The Subrecipient will be responsíble for administering the Rental Assistance program in a manner <br />satisfactory to the City and consistent with any standards required as a condition of providing these <br />funds. Such program will include the following activities eligible under the Community Development <br />Block Grant Program: <br />Program Delivery <br />Activitv: <br />The RentalAssistance program will serve 30 Persons by providing rental assistance <br />General Administration <br />[Description of general administrative seruices to be performed in support of activities noted above] <br />Hand in Hand's method is to build relationships with families living in poverty through on and offsite <br />direct activities, referrals from area schools, social service agencies, and the Department of Children, <br />Youth and Families. Using a case management approach, we will evaluate the family need, provide <br />dírect services and resources, and/or refer to and work with partner agencies for additional support. <br />Hand in Hand will monitor CDBG funds and review quarterly to ensure all funds are dispersed during <br />the reporting period. We anticipate providing $1,000 per household served.
