B. National Objectives <br />The Subrecipient certifies that the activities carried out with funds provided under this Agreement will <br />meet the following CDBG National Objective, as identified by the City of Everett for priority in the <br />2022 Program Year, "Benefit loMmoderate income persons"- as definedin24 CFR Part 570.208. <br />Appropriate subcategory and matrix code under the CDBG National Objectives: <br />Limited Glientele; 05Q, Subsistence Payments 24 CFR 570.207(bX4) or 42 USC 5305(a)(8) + <br />24 CFR 570.a82þl(21 <br />Description of how the project will meet this subcategory: For this grant, our Outreach team will <br />work with Everett families in need. Through our case management process, which includes <br />bi-weekly check-ins, review of circumstances and relationship building, we witl determine <br />ongoing ability to maintain current housing past the immediate need and connect client <br />with additional community resources which includes food banks, programs for low- <br />moderate income utility programs and feeding programs. We require proof of income <br />through pay stubs or a letter from employer/s for every adult living in the home. We require <br />proof of state issued identification such as a driver's Iicense for all adults living in the <br />home. We verify that all clients are LMI according to HUD AMI guidelines. <br />C. Performance Monitorinq <br />The City will monitor the performance of the Subrecipient against goals and performance standards <br />required herein through quarterly reports and on-site and/or virtual site visits. Substandard <br />performance as determined by the City will constitute non-compliance with this Agreement. lf action <br />to correct such substandard performance is not taken by the Subrecipient within a reasonable period <br />of time, as determined by the City and after being notified by the City, contract suspension or <br />termination procedures will be initiated.
