Page 20 of 31 <br />SECTION 4 – PROPOSAL SUBMITTAL REQUIREMENTS <br />4.1 SUBMITTAL REQUIREMENTS <br />Suppliers must provide a proposal which must demonstrate an understanding of the project requirements <br />as stated throughout this Request for Proposal. <br />Proposals in response to this RFP must be submitted in the order specified below. Proposal responses <br />must include: <br />1. Supplier Commitment and Information (included) <br />2. Price Sheet (included) <br />3. Narrative responses to the questions asked. Suppliers should re-type the heading, question <br />identifier and question. Then answer the questions and provide in the same order requested below. <br />Suppliers may emphasize in their narrative any areas of their proposal that they believe exceed our <br />requirements. <br />4. Certificate of Non-Debarment/Suspension (included) <br />5. Washington State Department of Natural Resources Clauses (included) <br />6. ARPA Clauses and Certifications (included) <br />4.2 SUGGESTED RESPONSE FORMAT <br />• Standard 8 1/2" x 11" paper <br />• Single or double sided, numbered pages <br />• Typed with a minimum of 12-point font <br />• Form 4.03 – re-type the question before responding <br /> <br />
