Exhibit "A" Scope of Services, Time of Performance, Budget, Payment, Notices, and Representatives I. Scope of Service A. Activities/Project Description The Subrecipient will be responsible for administering the Homelessness Prevention for Covid Relief program in a manner satisfactory to the City and consistent with any standards required as a condition of providing these funds. Such program will include the following activities eligible under the Community Development Block Grant Program: Program Delivery Activity: The Homelessness Prevention for Covid Relief will serve 73 Persons by providing Rental relief and homeless prevention. General Administration [Description of general administrative services to be performed in support of activities noted above] Intake, Case Management, Financial Literacy Training, Rental assistance and other approved supports to help family remain housed in their current residence. B. National Objectives The Subrecipient certifies that the activities carried out with funds provided under this Agreement will meet the following CDBG National Objective, as identified by the City of Everett for priority in the 2020 Program Year, "Benefit low/moderate income persons" - as defined in 24 CFR Part 570.208. Appropriate Subcategory and Matrix Code under the National Objective: Limited Clientele 05Q: Subsistence Payments 24 CFR 570.207(b)(4) or 42 USC 5305(a)(8) + 24 CFR 570.482(c)(2) - One-time or short-term (no more than three months) emergency payments on behalf of individuals or families, generally for the purpose of preventing homelessness. Description of how the project will meet this subcategory: This will provide one month of support for emergency payments for low-income families. Emphasis is on preventing eviction so the family will not move into homelessness. Income will be documented by source documentation such as recent paystubs, bank statements, or third party verification of income. C. Performance Monitoring
