The City will monitor the performance of the Subrecipient against goals and performance standards required herein through quarterly reports and site visits. Substandard performance as determined by the City will constitute non-compliance with this Agreement. If action to correct such substandard performance is not taken by the Subrecipient within a reasonable period of time, as determined by the City and after being notified by the City, contract suspension or termination procedures will be initiated. D. Levels of Accomplishment - Goals and Performance Measures In addition to the normal administrative services required as part of this Agreement, the Subrecipient agrees to provide the following levels of program services: Activity I Units of Service* per Month ' Total Units/Year Case manage and assist families affected by 16 individual assisted 73 individuals or 23 the COVID-19 situation remain housed in their per month. The Subrecipient will report the outcomes to the City on the following schedule: Performance Period Report Due July 1 , 2022 — December 31, 2022 January 31, 2023 January 1, 2023 — March 31, 2023 April 29, 2023 April 1, 2023 — June 30, 2023 July 29, 2023 Reporting forms must be in the format acceptable to the City of Everett.
