i <br /> Section 00 72 00 GENERAL CONDITIONS <br /> 7.2. PAYMENT CERTIFICATES <br /> A. The Contractor and each Subcontractor on or before the date of commencement of the Work shall <br /> file a statement of"Intent to Pay"prevailing wages under oath with the Owner and with the Washington <br /> State Department of Labor and Industries certifying the rate of hourly wage including the usual benefits <br /> paid and to be paid each classification of laborers,workmen,or mechanics employed upon the Work by <br /> the Contractor or its Subcontractor that shall not be less than the prevailing rate of wage. No payment <br /> will be made to the Contractor prior to the submission of such statements and the issuance by the <br /> Industrial Statistician of said Department of an acknowledgment of approval. Such statement and any <br /> supplemental statements that may be necessary shall be filed in accordance with the practices and <br /> procedures required by the Department of Labor and Industries. Upon the completion of the Work,the <br /> Contractor and all of its Subcontractors shall submit affidavits of wages paid to the Department of Labor <br /> and Industries in such form as may be required by said Department. Payment of the retained percentage <br /> will not be made until certification is received from the Department of Labor and Industries that the <br /> prevailing wage requirements of state law have been satisfied. <br /> B. Any fees charged by the Department of Labor and Industries for filing such statements or <br /> submitting such affidavits shall be the responsibility of the Contractor,and each Subcontractor;if,for any <br /> reason,the Owner pays such fees,then the Contractor shall be charged the amounts thereof. <br /> C. Unless otherwise determined by Owner,the Owner will require payroll reports for the Contractor <br /> and every Subcontractor be submitted weekly to the Construction Division,Public Works Annex, 3101 <br /> Cedar Street,Everett, Washington 98201. The payroll reports shall contain the following information: <br /> 1. Name and residence address of each worker. <br /> 2. Social Security number of each worker. <br /> 3. Classification of work performed by each worker. The classification shall be specific and match <br /> the classification categories listed in the Contract Documents. <br /> 4. Total number of hours employed each day. <br /> 5. Total number of hours employed during the payroll period. <br /> 6. Straight time and overtime hourly rate of wages paid to each worker. <br /> 7. Total or gross amount earned by each worker. <br /> 8. Deductions for Medical Aid,FICA,Federal withholding tax,and any other deductions taken. <br /> 9. Net amount paid each worker. <br /> 10. Contractor's, or Subcontractor's,name and address. <br /> 11. Days and dates worked. <br /> 12. Date of final day of pay period. <br /> 13. Whether fringe benefits were paid to each worker as part of the hourly wage rate or whether <br /> fringe benefits were paid into an approved plan, fund,or program. <br /> Payrolls may be submitted on Federal payroll form WH-347,or equivalent. The reverse side of the form <br /> contains an affidavit that shall be filled out and signed. If the Contractor's payroll reports are <br /> computerized, the computerized reports may be submitted along with a Statement of Compliance affidavit <br /> photo copied from the back of form WH-347,or equivalent. <br /> ' The first payroll submitted for the Work for both the Contractor and each Subcontractor shall be labeled <br /> "Initial." The last payroll submitted for the Work for both the Contractor and each Subcontractor shall be <br /> labeled"Final." Payrolls shall be sequentially numbered for all periods in which Work has been done. A <br /> certificate of completion for the Work,signed by the City,will constitute acceptance of the Work.
