Section 00 21 13 INSTRUCTIONS TO BIDDERS <br /> 5. Statement of Intent to Pay Prevailing Wage (L&I Form F700-029-000) and Affidavit of <br /> Wages Paid (F700-007-000)from the Contractor, Subcontractor and agents to the <br /> Subcontractor shall be submitted to the Employment Standards Division, State <br /> Department of Labor and Industries, Olympia, Washington. If the prime contract is over <br /> $1,000,000, then Contractor also needs to submit L&I Form F700-164-000 (Affidavit of <br /> Wages Paid EHB 2805 Addendum). <br /> 6. Weekly Statement with Respect to Payment of Wages (U.S. Dept of Labor Form <br /> WH347): Contractors, Subcontractors, and agents to Subcontractors using Payroll Form <br /> WH347 may use State of Compliance found on the back of the form. <br /> 7. Weekly Statement of Compliance (U.S. Dept of Labor Form WH348). Contractors, <br /> Subcontractors, or agents to Subcontractors not using Payroll Form WH347 shall attach <br /> the Statement of Compliance Form WH348 to each payroll. <br /> 8. Approval of Subcontractors: Contractors shall request approval of Subcontractors on a <br /> form provided by the City prior to their working on the Site. <br /> 9. Construction Progress Schedule: To be submitted as required by Section 00 7200, Part I <br /> 5 "Progress and Completion", of the Contract Documents by the Contractor within ten <br /> calendar days, or such time as determined by the City, after the date of receipt of Notice <br /> to Proceed. <br /> 10. Traffic Control Plan: Standard Traffic Control Plans are included in the City's Design and <br /> Construction Standards. If, and to the extent, such plans are not sufficient, complete or <br /> adequate to support the Bidder's planned means and methods of performing the Work, <br /> the Bidder must develop an adequate, complete and sufficient traffic control plan at its <br /> cost, that shall be submitted for the City's approval prior to construction. Deviation from <br /> the Standard Plans must be submitted by the Contractor and approved by the City prior <br /> to construction. <br /> 11. Disposal Sites: Provide the City with the location of all disposal sites to be used, and <br /> also provide copies of the permits and approvals for such disposal sites.
