written Construction Change Directive prepared by the Architect and authorized/signed <br /> by the Owner's Representative. <br /> D. Change Order Pricing: <br /> 1. The cost of the change shall be marked-up in accordance with the General <br /> Conditions. No additional mark-ups shall be allowed. <br /> 2. Contractor shall provide all backup pricing documentation for a change on the <br /> following forms (These forms shall also be the only acceptable documentation <br /> for all subcontractors.): <br /> a. PR GC/CM Breakdown Summary. <br /> b. PR Subcontractor Breakdown Summary. <br /> c. PR Cost Breakdown. <br /> 3. Owner or Architect may require Contractor to provide certified payroll. <br /> 4. Provide all other supporting documentation as required to substantiate the <br /> requested costs such as invoices for rental equipment and freight cost. Total cost <br /> and time shall be brought forward to the PR form and signed and dated by <br /> Contractor. <br /> E. Change Order Authorization: <br /> 1. Architect's recommendation of PR acceptance is indicated by Architect's signature. <br /> 2. Upon signature and execution by Owner, the Proposal Request will be incorporated <br /> into a Change Order altering the Total Contract Cost and/or Contract Time, as <br /> indicated. <br /> 3. Contractor may only request payment for changes in the Work against an approved <br /> Change Order. <br /> 4. If Owner disapproves the Proposal Request, the reason for disapproval will be stated. <br /> A request for a revised proposal or cancellation of the proposal will be shown and <br /> returned to Contractor. <br /> F. Correlation with Contractor's Submittals: <br /> 1. Application of Payment forms shall record each Change Order as a separate item <br /> of work (see Section 01 29 76 "Progress Payments- GCCM"). <br /> 2. Revise Construction Progress Schedule to reflect changes in Contract Time. <br /> 3. Upon completion of Change Order work, record pertinent modifications in the Project <br /> Record documents. <br /> G.
