EXCLUSIONS <br /> Your attention is directed to the attached MNW Terms and Conditions. Please pay particular attention to the <br /> items indicated below. <br /> 1. Un-obstructed access to suitable laydown areas and all areas of work. <br /> 2. Removal and disposal of drill tailings and drill spoils. <br /> 3. Inspection, observation, or testing other than testing specifically identified above. <br /> 4. Sediment and erosion control measures or assessments. <br /> 5. Easements, as may be required for anchor installation. <br /> 6. Environmental impact assessments, reviews or remediation. <br /> 7. Hazardous materials encountered or uncovered during the process of our work. <br /> 8. Location and removal of any utilities that may be present in the work zone. To include potholing of <br /> local utilities. <br /> 9. Layout of vertical element beam locations/elevations,soil nail locations/elevations as well as <br /> locations/elevations of shotcrete wall. Layout to be provided on a continuous basis without delay. <br /> 10. Mass excavation and face cutting of soil nail walls—to include benching and acceptable access to <br /> enter and exit excavation. <br /> 11. Dumpsters and sanitation facilities are to be located not more than 200'from MNW work area. <br /> 12. Sampling,coring,testing, or inspection of grout,shotcrete, core samples. <br /> 13. Flaggers,street use permits or traffic control that may be required for the progress of our work or for <br /> the delivery of equipment or materials. <br /> 14. Shotcrete in excess of 185 CY will be invoiced at a rate of$235/CY. <br /> 15. Costs associated with de-watering to include hookup of weep drains to discharge systems. <br /> 16. Removal of installed shoring components—to include top portion of vertical elements. <br /> 17. Top of wall fall protection. <br /> 18. Capping or plugging of side sewers. <br /> 19.
