EXHIBIT A <br />SCOPE OF WORK <br /> <br />Exhibit A to Professional Services Agreement <br />(Form Approved by City Attorney’s Office January 7, 2010, updated December 2, 2022) <br /> <br /> <br />The City of Everett Main Library is an approximately 111,625-square-foot, 2-story structure with three <br />levels of partial below-grade parking circa 1923. Over the years, the library has gone through multiple <br />renovations, additions, upgrades, and repairs. <br /> <br />The purpose of this contract is to provide design, architectural and engineering services associated with <br />exterior building envelope improvements to the Everett Main Library located at 2702 Hoyt Avenue, <br />Everett, WA 98201. <br /> <br />OAC Services, Inc. will complete a condition assessment of the building’s roof(s) and facades <br />documenting where failure has occurred resulting in water intrusion to the interior. Areas of assessment <br />include the building roofs, parapets caps, windows below the barrel roof, flashing of windows, and <br />sealant replacement including terracotta roof cap, slate tile, and brick façades. <br /> <br />In addition, OAC will develop a scope of repair to address deficient conditions, design the repairs for the <br />reroofing of the Main Library, support the Parks & Facilities Department with permitting and bidding, <br />include a preliminary estimate for associated construction costs and provide support during the <br />construction process. <br /> <br /> <br />1) PREDESIGN – SITE OBSERVATION AND ASSESSMENT <br /> <br />• Mobilization – Project and contract management <br />• Review of existing documents including as-built drawings, and maintenance records that are <br />made available <br />• Conduct a visual site observation and intrusive investigation at the facility to document and <br />gather information about the existing condition of the roof at failure locations. <br />• Coordination with Client/Contractor via conference call. <br /> <br />2) DESIGN DEVELOPMENT – 60% DD Set <br /> <br />• Develop a 60% DD drawing set (PDF format) for the required repairs for permitting <br />purposes.
