restraint during assembly. Standard guardrails consist of 42" top -rail securely coupled to a post and <br />a 21" mid -rail securely coupled to the post. <br />Thejob supervisor will perform a daily inspection of the standard guardrail system at the beginning <br />of each work shift. Any maintenance will be completed during that work shift. When forms are <br />being moved, handrails will be temporarily removed until the form is repositioned. At that point, <br />handrails will be promptly replaced. <br />Whenever possible, the standard guardrail system will be disassembled in an area where the fall <br />hazard does not exist. IFthis is not possible, employees performing disassembly will be protected <br />with a standard body harness system. <br />Standard Body Harness <br />Each employee will be trained in the use of a standard body harness. This instruction will include <br />daily inspection for: <br />• Mildew <br />• Wear <br />• Damage <br />• Deterioration <br />• Defective or missing components <br />Lifelines and Lanyards <br />Employees will learn how to assemble/disassembl a lifelines and attach lanyards and fall restraint <br />devices as needed. Anchorages must be sufficient to support a 5000 lb. load. Examples of <br />acceptable anchorages include steel columns, anchor bolttemplates, rebar dowels, and stationary <br />equipment. All lifelines and lanyards shall be assembled in such a manner as to prevent them from <br />being cut or abraded. <br />When vertical lifelines are used, not more than one employee shall be attached to a single lifeline at <br />a time. <br />Lifelines and lanyards must be inspected prior to each use. Employees should look for signs of <br />excessive wear, damage or deterioration that could adversely affect the strength. Employees that <br />notice defects should notify thejob supervisor immediately and not use the device. <br />Daily Inspections <br />A visual inspection of all safety equipment is required daily or before each use. Defective <br />equipment is tagged and removed from use immediately. The manufacturers' recommendations <br />for maintenance and inspection and for assembly of equipment must be followed. <br />Overhead Protection and Handling/Securing of Tools <br />Hardhats are required on all jobsites. Warning signs, barricades, and warning tape must be posted <br />to caution workers of existing hazards when these are present. All floor openings must be covered <br />with adequate coverings, guardrails installed, orthey must be constantly attended. Work platforms <br />will have to boards to prevent large tools from falling to a work area below. In addition, the area <br />below the platform may be taped off to prevent access. <br />
