Section 18: For-hire driver's license-Application. A. Applicants for for-hire driver's licenses shall furnish the following information on forms to be furnished by the city, which information shall be signed and sworn to: 1. The applicant's full name and any aliases; 2. The applicant's business address at the time of application and residence address(es) at the time of application and for a period of two years prior thereto; 3. Residence and business telephone numbers; 4. Place and date of birth; 5. Height, weight, color of eyes and hair; 6. Proof that the applicant is a United States citizen or has documentation, as required by the United States Department of Justice Immigration and Naturalization Service,that the applicant is authorized to work in the United States; 7. Employment history for two years preceding the date of application, including name of employer,beginning and ending dates of employment, address,telephone number and name of supervisor; S. Information indicating whether or not the applicant has been previously licensed as a driver or chauffeur and if so, when and where 9. Information indicating whether or not the applicant has ever had a for-hire or driver's license suspended, revoked, or denied, and for what cause; 10. Washington State driver's license number. The applicant must present his/her Washington State driver's license at time of renewal; 11. Whether the applicant has ever had a conviction or bail forfeiture involving a crime of moral turpitude, a criminal traffic offense, or a crime involving alcohol, controlled substances, prostitution, gambling, dishonesty or physical violence; 12. Such other information the city clerk may require, which he/she deems reasonably necessary to aid in the enforcement of this ordinance. B. The application shall be filed with the city as a permanent record and shall contain the name and full address of two persons, not relatives, who have known the applicant for at least two years past. Section 19: For-hire driver's license- Qualifications. A. The operation of a for-hire vehicle business is a privilege, not a right. An applicant or licensee's ability to satisfy stated criteria for a for-hire vehicle business license does not create a right to a for-hire driver's license. B. A for-hire driver's license will be denied unless the applicant possesses the following qualifications: 1. Must be at least eighteen years of age. 2. Must possess a valid state of Washington motor vehicle operator's license; 3. Must be able to read, speak and write the English language; 4. Must be free from any infirmity of the body or mind which would render the applicant unfit for safe operation of a motor vehicle; 5. Must not be found to be addicted to the use of intoxicating liquor, dangerous drugs or narcotics within two years of the date of application; 10
