Section 18: For-hire driver's license-Application. <br /> A. Applicants for for-hire driver's licenses shall furnish the following information on <br /> forms to be furnished by the city, which information shall be signed and sworn to: <br /> 1. The applicant's full name and any aliases; <br /> 2. The applicant's business address at the time of application and residence <br /> address(es) at the time of application and for a period of two years prior thereto; <br /> 3. Residence and business telephone numbers; <br /> 4. Place and date of birth; <br /> 5. Height, weight, color of eyes and hair; <br /> 6. Proof that the applicant is a United States citizen or has documentation, as <br /> required by the United States Department of Justice Immigration and <br /> Naturalization Service,that the applicant is authorized to work in the United <br /> States; <br /> 7. Employment history for two years preceding the date of application, including <br /> name of employer,beginning and ending dates of employment, address,telephone <br /> number and name of supervisor; <br /> S. Information indicating whether or not the applicant has been previously licensed as <br /> a driver or chauffeur and if so, when and where <br /> 9. Information indicating whether or not the applicant has ever had a for-hire or <br /> driver's license suspended, revoked, or denied, and for what cause; <br /> 10. Washington State driver's license number. The applicant must present his/her <br /> Washington State driver's license at time of renewal; <br /> 11. Whether the applicant has ever had a conviction or bail forfeiture involving a crime <br /> of moral turpitude, a criminal traffic offense, or a crime involving alcohol, <br /> controlled substances, prostitution, gambling, dishonesty or physical violence; <br /> 12. Such other information the city clerk may require, which he/she deems reasonably <br /> necessary to aid in the enforcement of this ordinance. <br /> B. The application shall be filed with the city as a permanent record and shall contain <br /> the name and full address of two persons, not relatives, who have known the <br /> applicant for at least two years past. <br /> Section 19: For-hire driver's license- Qualifications. <br /> A. The operation of a for-hire vehicle business is a privilege, not a right. An applicant <br /> or licensee's ability to satisfy stated criteria for a for-hire vehicle business license <br /> does not create a right to a for-hire driver's license. <br /> B. A for-hire driver's license will be denied unless the applicant possesses the <br /> following qualifications: <br /> 1. Must be at least eighteen years of age. <br /> 2. Must possess a valid state of Washington motor vehicle operator's license; <br /> 3. Must be able to read, speak and write the English language; <br /> 4. Must be free from any infirmity of the body or mind which would render the <br /> applicant unfit for safe operation of a motor vehicle; <br /> 5. Must not be found to be addicted to the use of intoxicating liquor, dangerous <br /> drugs or narcotics within two years of the date of application; <br /> 10 <br />
