Hayden LeBlonde
From: Nick Welling <Nick.Welling@psengineers.com>
Sent: Tuesday, February 21, 2023 5:42 PM
To: Hayden LeBlonde
Cc: Paul Gentsch; Kiel Lunsford; Ike Burkett;John Brohard; Bill Sandbo; Mackenzie Lostra
Subject: RE: Helion - Final Approvals - Mat Foundation Pour 2/22
Attachments: Re: Helion Form Savers Adjustments; 2023_02_21 SO #4 Final Foundation Observation
(32).JPG; 2023_02_21 SO #4 Final Foundation Observation (43).JPG
Hi All,
Thanks for having us on site today. Following up on Mackenzie's previous email with the latest from Structural
Observation #4. Note that we are working on putting together a formal report, but wanted to get you a follow up email
ASAP since the pour is scheduled for tomorrow morning. For our latest comments and action items, see content from
Mackenzie's previous email below with SO#4 updates/additions in BLUE:
Outstanding items from SO#1 report included:
- Some instances of bent bars near the column footing details were observed to be laid on their sides
at the time of the visit on 1/31/23; these appeared to be resolved.—Previously confirmed/resolved
- Installation of the#8 bar pairs had been partially complete at the time of the visit on 1/31/23;
installation appeared to be completed. - Previously confirmed/resolved
- All improper lap splices appeared to be resolved. - Previously confirmed/resolved
- A majority of the mylar sheets and tie wire scattered throughout the bottom of the footing
appeared to be removed. Some minor instances still remain and a final clean-up should be done
prior to the pour as much as feasibly possible.—By time of SO#4, mylar sheets appeared to be
mostly cleaned up. PSE still recommends doing a final clean-up prior to the pour.
Outstanding items from SO#2 report included:
- Major congestion due to the many lap splices of the#11 bars within the rebar cage region appeared
to be significantly reduced following removal of the#11 bar lines as outlined in the SO#2 report and
RFI #008.All anchors (one exception discussed in following section)were present and in place,
which was the primary concern with the congested bars along the transverse sides of the footing.—
Previously confirmed/resolved
- Previous notes of bar clearance violations appeared to be resolved. No further instances were
noticed.—Previously confirmed/resolved
- Some minor instances of loose weather barrier were still observed. Any instances of loose form liner
or weather barrier should be re-established prior to the pour.—During SO#4,the crew was working
to resolve the outstanding instances.
- Similar comment on minor instances of mylar sheet and tie wire remnants.
Items discussed during SO#3:
- Instances of the#5 stirrup bars with hooked legs that were cut-off or cut-short shall be replaced
with bars with proper hooks or, if replacement is not possible, lapped with "L" bars. Laps should be
tied sufficiently and splice lengths should be maximized. This was discussed on site with Andersen
and remedies were underway. See attached photo#1.—Not all instances had been corrected by
time of SO#4. Crew mobilized to begin installing some additional bars to correct for instances of
cut-off hooked bar legs noted above.
- One instance of missing anchor bars was noticed on the west side of the foundation. Space for full
anchor bar pair was minimal, however, stirrup bars without hooks (vertical leg only) should be
1
