Swap out the wood key blockouts with the replacement pieces and re-set into place prior to the <br /> pour.—Corrected keyway blockout forms were observed and appeared to be in general <br /> conformance with PSE's plans. <br /> The keys were not noticed to be treated with form oil, but new pieces should be treated as <br /> necessary to ensure easy removal.—Comment remains applicable. If not done so already,treat as <br /> necessary to ensure easy removal. <br /> Final efforts should be made to protect exposed threads (in form savers and at the anchor rod <br /> couplers) prior to the pour.Absolutely no chances for concrete to penetrate the form savers of the <br /> pilaster bars should be tolerated.—At time of SO#4, work was ongoing to pack formsavers with <br /> foam and cover with duct tape. Ensure all formsavers are fully protected prior to the pour. <br /> Templates and form placement should be completed and should be verified that pressure of the wet <br /> concrete will not cause these to move out of plumb.—Comment remains applicable. <br /> All top of anchor coupler elevations should be checked a final time prior to the pour.—Comment <br /> remains applicable <br /> - Install or replace the#5 stirrup bars where insufficient, this was underway while on site. Comment <br /> remains applicable. See previous notes above regarding ongoing and previously completed <br /> corrections. <br /> Re-establish any loose form liner or weather barrier to the sides of the foundation prior to the pour. <br /> —At time of SO#4, work was ongoing to re-establish as noted. <br /> Remove unwanted objects within foundation area (mylar, tie wire, concrete blocks, steel rod, etc.)— <br /> Comment remains applicable. Review base of slab,thickened footing regions, and atop the existing <br /> column footings prior to pouring concrete. <br /> PSE and representatives from Andersen discussed the items above on site during SO#4. Please ensure the corrections <br /> and remaining action items communicated on site and noted above are addressed prior to the pour tomorrow.
