Helion Energy Shielding Structure -Foundation February 24, 2023 <br /> Helion Energy <br /> The following items have been verified as corrected in field and which were noted as out-of-conformance items <br /> in Structural Observation Report#1: <br /> 1. (Resolved) — Instances of bent bar reinforcement near the column footing cages were observed to be <br /> laid on their sides at the time of the visit and appeared to be corrected at the time of SO#4. <br /> 2. (Resolved)— Installation of the#8 bar pairs for the anchor reinforcing that had been partially complete <br /> at the time of the site visit on 1/31/23 appeared to be completed. <br /> 3. (Resolved)—All instances of improper lap splices were resolved. <br /> The following items have been verified as corrected in field and which were noted as out-of-conformance items <br /> in Structural Observation Report#2: <br /> 1. (Resolved)—Major congestion due to adjacent lap splices within the rebar cage regions appeared to be <br /> significantly reduced following removal of the#11 bar lines as outlined in SO#2 and RFI#008.All anchors <br /> (except for one instance—see Action Item#3)were present and in place,which was the primary concern <br /> with the congested bars along the transverse sides of the foundation. <br /> 2. (Resolved) — Previous notes of bar clearance violations appeared to be resolved. No further instances <br /> were noted. <br /> 3. (Resolved) — Some minor instances of loose weather barrier were still observed. Any outstanding <br /> instances of loose form liner or weather barrier should be re-established prior to the concrete pour. <br /> Additional Notes/In-Conformance Items: <br /> 1. General layout of pilaster dowels and hook orientation was acceptable. <br /> 2. Pilaster dimensions and global location were acceptable. <br /> 3. Wood keyways did not appear to be treated with form oil but should be treated as necessary to ensure <br /> easy removal. <br /> 4. Anchor rod spacing at the template elevation were measured in field and appeared to be in conformance <br /> with the anchor layout in the plans. General plumb-ness of the anchors was verified in field and <br /> appeared to be in conformance with the plans. Some instances of template forms not yet secured in <br /> place were observed. All forms shall be secured in place prior to the pour to ensure the suspended <br /> anchors do not move out of plumb under the pressure of wet concrete. <br /> 5. PSE understands that cold weather conditions may potentially impact the concrete pour on the <br /> scheduled pour date of 2/22/23. It was discussed with Andersen that thermal blankets would be utilized <br /> during the curing process if cold weather conditions persist. <br /> 6. PSE understands that the boron carbide additive to the concrete mix would be done on site on the day <br /> of the concrete pour. Andersen intends to keep detailed log of all quantities of admixtures included in <br /> each truck load. <br /> 7. Andersen informed PSE that they intend to wet set the preformed joint filler at the perimeter of the new <br /> mat foundation. <br /> Hayden Leblonde of Andersen Construction was on site at the time of these observations and was notified of <br /> the above-mentioned action items. The additional notes were briefly discussed but have been expanded upon <br /> in this report with supplemental information. Once final action items are addressed, PSE finds the structural <br /> elements to be in substantial compliance with the structural contract documents in anticipation of the scheduled <br /> concrete pour. <br /> PAGE 1 3 <br />
