Action Item 3 (Resolved) — It was noted that the wood forms for the keyway blockouts did not match the <br /> geometry per the contract plans. Possible solutions were discussed in field; however, it was agreed that the <br /> best solution was to reconstruct the key forms to the geometry shown on the plans.The corrected keyway <br /> blockouts had been installed at the time of SO#4 and were in conformance with the plans. See Figure 4. <br /> 4. Action Item 4 (Resolved)—At the adjacent building column footing locations, the longer threaded rods with <br /> large steel plate washer(6"x 6")was noted to be elevated above the Styrofoam layer,which deviated from <br /> detail 4/SF6 of the contract plans, however, PSE finds this acceptable, and no adjustment is required prior <br /> to the concrete pour.The change should be documented in the as-built drawings. See Figure S. <br /> 5. Action Item 5 (Resolved) — Final covers/protection had not yet been installed in the form savers of the <br /> pilaster dowels by S03.Andersen was notified that final efforts should be made to protect exposed threads <br /> to ensure ease of installation of the future pilaster reinforcement bars. At SO#4, workers were packing the <br /> form savers with foam and duct tape covers.All instances appeared to be resolved. See Figure 6. <br /> 6. Action Item 6—Instances of irregular elevation and angle of the pilaster dowels and top of form savers were <br /> observed at most of the pilaster locations during SO#3. Some slight inconsistencies in elevation and plumb- <br /> ness remained during SO#4. Further review and correction should be made by Andersen to the greatest <br /> extent possible prior to the concrete pour to correct the irregularities and ensure ease of installation of the <br /> future pilaster reinforcement bars. In particular, instances of form savers at low elevations should not be <br /> tolerated. Instances of form savers at elevations slightly above measured grade are acceptable and may be <br /> protected with pack grout following the pour. See Figure 7. <br /> 7. Action Item 7—One instance was noted in the northwest corner of the foundation where both #5 anchor <br /> reinforcing bars were placed on the same side of a cast-in anchor.The bars shall be relocated such that they <br /> are on either side of the anchor bar. Andersen was notified of the correction while on site during SO#4 and <br /> was working to correct the error. See Figure 8. <br /> 8. Out-of-Conformance Item 1 (Resolved)—Outstanding remnants of tie wire and mylar sheets that had fallen <br /> to the bottom of the excavated slab area appeared to be sufficiently cleared at the time of SO#4. <br /> 9. Out-of-Conformance Item 2 (Resolved)—One instance of a large concrete block noted near the rebar cage <br /> region at the SW corner of the foundation, which was noted during SO#3, appeared to be removed at the <br /> time of SO#4. <br /> 10. Out-of-Conformance Item 3 (Resolved) — One instance of a large steel rod noticed near the middle of the <br /> foundation on the south edge, which was noted during SO#3, appeared to be removed at the time of SO#4. <br /> 11. Out-of-Conformance Item 4 (Resolved)— Instances of concrete dobies no longer used for maintaining clear <br /> cover,which was noted during SO#3, appeared to be removed at the time of SO#4. <br /> PAGE 12 <br />
