CITY OF EVERETT SPECIAL PROVISIONS <br /> <br />Beverly Lake Water Quality RetrofitDivision B – BID DESCRIPTIONS August 2023 <br />WO No. – UP3745 SP - 33 <br /> <br />of HMA – CL 1/2” PG 64-22 pavement including streets, alleys, street crossings, and driveway <br />aprons as shown on the Plans. <br />Miscellaneous HMA used for pavement repair, thickened edges, extruded and wedge <br />curbing, placing and compacting pre-leveling, leveling and wearing courses, feathering <br />pavement is included in this bid item with no direct compensation made. <br />Furnishing and applying tack coat, prime coat, joint seal and crack sealing asphalt is to be <br />included in this bid item with no direct compensation made. All costs for “Anti Stripping <br />Additive” and “Compaction Adjustment” shall be included in the unit contract price per ton of <br />the HMA with no direct compensation made. <br />The unit price for this bid item shall include all labor, materials, equipment and related work <br />necessary to furnish, place, compact, roll and perform density tests on HMA placed in <br />accordance with the Plans, Standard Specifications, and these Special Provisions. <br /> <br />Bid Item 28 - Cement Concrete Sidewalk Restoration <br />Measurement and Payment: Unit Price per Square Yard (SY) <br />Measurement shall be per square yard as delineated by the Contractor and approved by the <br />Inspector and adjacent property owner prior to removal. <br />The unit price per square yard for restoration of existing concrete sidewalks shall be full <br />compensation for final sawcut and all materials, tools, labor and equipment necessary to <br />complete the work in accordance with the Plans, COE Standard Drawings, Standard <br />Specifications and these Special Provisions. <br />Bid Item 29 - Cement Concrete Curb and Gutter, Type A-1 <br />Measurement and Payment: Unit Price per Linear Foot (LF) <br />Measurement shall be per linear foot as delineated by the Contractor and approved by the <br />Inspector prior to removal. <br />The unit price per linear foot for restoration of concrete curbs or curb and gutter and shall be <br />full compensation for all materials, tools, labor and equipment necessary to complete the <br />Work in accordance with the Plans, COE Standard Drawings, Standard Specifications and <br />these Special Provisions, including sawcutting, removal, loading, hauling and disposal. <br />Bid Item 30 - Cement Concrete Driveway, Type 1 <br />Measurement and Payment: Unit Price per Square Yard (SY) <br />Cement concrete driveway entrances will be measured by the square yard of finished <br />surface. <br />The unit price per square yard for cement concrete driveway, type 1 shall be full <br />compensation for final sawcut and all materials, tools, labor and equipment necessary to <br />complete the work in accordance with the Plans, COE Standard Drawings, Standard <br />Specifications and these Special Provisions. Installing and removing any temporary <br />approach shall be included. <br /> <br />Bid Item 31 - Extruded Cement Concrete Curb <br />Measurement and Payment: Unit Price per Linear Foot (LF) <br />Measurement shall be per linear foot as measured along the face of the curb. <br />The unit price per linear foot for Extruded Cement Concrete Curb shall be full compensation <br />for all materials, tools, labor and equipment necessary to complete the Work in accordance <br />with the Plans, COE Standard Drawings, Standard Specifications and these Special
