STATE OF WASHINGTOTT County of Snohomish se being first duly sworn upon oath, deposes and says: I am the above named applicant, and make this affidavit for the purpose of obtaining from the City of Everett a license in accordance with the provisions of the License Code. I have personal knowledge of the matter stated in the foregoing application and the statements contained therein are true. Subscribed and sworn to before me this day of 19_..._ Notary Public in and for the State of Washington, residing at Everett. All applications for licenses under the provisions of this ordinance shall be filed with the City Clerk, except applications for the following licenses, which shall be filed with the City Council. (1) Billiard and Pool'`Table Licenses. (2) Card Room Licenses. (3) Dance Hall Licenses. (4) Employment Agency Licenses. Section 12 t Tie City Council upon presentation of any application for any of the licenses specified in Section 11 of, this ordinance, and before acting upon the same, shall request the Chief of Police to make a full investigation as to the truth of the statements contained therein, and as to any and all. other matters which might tend to aid the City Council in determining whether or not such application should be granted. The Chief of Police shall, within five (5) days after the date of such notice and request, furnish a written report to the City Council containing the result of his investigation; provided, how- ever, that all matters pertaining to an application for a dance Hall License shall be referred by the Chief of Police to the Police Woman for investigations and report. If the City Council be satisfied that the statements contained in such application are true, that the applicant and all persons con- nected With the business for which a license has been applied for are of good moral character, and that the premises described in the application comply with the requirements of the ordinances of the City of Everett relating to buildings and health and sanitation, and when all other requirements relating to the business for which a license has been applied for have been complied with,' the City •Council, shall, direct the City Clerk to issue the license applied for in con- formity with the provisions of this ordinance; provided, ho , ever, that no such license shall be granted in the event that the applicant (if a corporation, any of its officers) has ever been convicted of a felony or a misdemeanor involving moral turpitude or an intent to defraud or in the event that the applicant has been convicted of a violation of the ordinances of the City of Everett, the laws of the State of Washington, or the United States pertaining to the manufacture, keeping, use or transportation of intoxicating liquor or in the event that the City Council has good reason to believe that such persons are not of good moral character, or that such persons desire such license to enable them to engage in some dishonest, unlawful, or immoral act or practice; 1
