City of Everett 00 2113 - 14 <br />2024 WPCF Biosolids Removal UT 3826-2 <br /> <br /> <br />00 2113 - 14 INSTRUCTIONS TO BIDDERS <br />Forms for Submission after Award of Contract <br />The following forms are to be executed after the award of Contract: <br />1. Contract: This Contract to be executed by the successful Bidder in <br />duplicate within 14 calendar days after the award date. <br />2. Payment and Performance Bonds: These forms are to be executed by the <br />successful Bidder and his surety company in triplicate. The amount of these <br />bonds shall be 100 percent of the Total Bid and shall be submitted with the <br />Contract. <br />3. Proof of Insurance: Insurance certificates and endorsements shall be <br />obtained and maintained in force in accordance with SECTION 00 7200, <br />Part 15 “Liability and Insurance”, of the Contract Documents. <br />4. Power of Attorney: Attorneys-in-fact who sign bid bonds or contract bonds <br />must file with each bond a certified and effectively dated copy of their Power <br />of Attorney. <br />5. Statement of Intent to Pay Prevailing Wage (L&I Form F700-029-000) and <br />Affidavit of Wages Paid (F700-007-000) from the Contractor, Subcontractor <br />and agents to the Subcontractor shall be submitted to the Employment <br />Standards Division, State Department of Labor and Industries, Olympia, <br />Washington. If the prime contract is over $1,000,000, then Contractor also <br />needs to submit L&I Form F700-164-000 (Affidavit of Wages Paid EHB 2805 <br />Addendum). <br />6. Weekly Statement with Respect to Payment of Wages (U.S. Dept of Labor <br />Form WH347): Contractors, Subcontractors, and agents to Subcontractors <br />using Payroll Form WH347 may use State of Compliance found on the back <br />of the form. <br />7. Weekly Statement of Compliance (U.S. Dept of Labor Form WH348). <br />Contractors, Subcontractors, or agents to Subcontractors not using Payroll <br />Form WH347 shall attach the Statement of Compliance Form WH348 to <br />each payroll. <br />8. Approval of Subcontractors: Contractors shall request approval of <br />Subcontractors on a form provided by the City prior to their working on the <br />Site. <br />9. Construction Progress Schedule: To be submitted as required by Section <br />00 7200, Part 5 “Progress and Completion”, of the Contract Documents by <br />the Contractor within ten calendar days, or such time as determined by the <br />City, after the date of receipt of Notice to Proceed. <br />10. Traffic Control Plan: Standard Traffic Control Plans are included in the City’s <br />Design and Construction Standards. If, and to the extent, such plans are <br />not sufficient, complete or adequate to support the Bidder’s planned means <br />and methods of performing the Work, the Bidder must develop an adequate, <br />complete and sufficient traffic control plan at its cost, that shall be submitted <br />for the City’s approval prior to construction. Deviation from the Standard <br />Plans must be submitted by the Contractor and approved by the City prior <br />to construction.
