Statement of Work <br />10/24/2023 Statement of Work Page 6 <br />•Email the document to their contact ID’s <br />•Submit forms <br />•Ability to e-Sign forms <br />Everett’s Defendant <br />•View forms made available by Clerk <br />•Ability to e-Sign electronic forms <br />ASSUMPTIONS <br />OMIGA Solutions assumes the following: <br />1.Everett will continue to work with OMIGA Solutions to resolve any <br />roadblocks associated with the product. <br />2.OMIGA Solutions will be providing the document storage integration per <br />agreements with Everett. <br />3.OMIGA Solutions will continue to develop as much of the solution as <br />possible on its own servers so that enhancements, feature upgrades, and <br />bug fixes can be shared with all other customers of OMIGA Solutions. <br />provided separately by OMIGA Solutions <br />4.The court agrees to pay the annual maintenance fee as outlined in the <br />SLA (Service Level Agreement). The SLA outlines the costs and <br />expectations associated with maintaining the software and data-integrity <br />standards imposed on OMIGA Solutions by the Administrative Office of the <br />Courts (AOC) and Everett’s own standards. <br />Security Agreement <br />In the case of a data breach of personal customer information through OMIGA <br />Solutions Inc.’s systems, OMIGA Solutions Inc. would be responsible for <br />notifying customers of the data breach. <br />Costs and Payment Schedule <br />•Everett agrees to fund $38,000 for the total effort due within thirty days <br />after implementation of the services to Everett Municipal Court. <br />•After the first year of service, Everett agrees to pay $9,000 annually, the <br />maintenance fee, associated with server maintenance and systems needs as <br />outlined in the Service Level Agreement.
