CITY OF EVERETT - CITYWIDE BICYCLE WAYFINDING <br />STATE FUND# HLP-PB21(014) <br />WO#3786 September 25, 2023 <br /> 38 <br /> 1 <br />All entities listed under 1-07.18(2) of these Special Provisions shall be named as additional 2 <br />insureds on the Contractor’s Excess or Umbrella Liability insurance policy. 3 <br /> 4 <br />This requirement may be satisfied instead through the Contractor’s primary Commercial 5 <br />General and Automobile Liability coverages, or any combination thereof that achieves the 6 <br />overall required limits of insurance. 7 <br /> 8 <br />COE 1-07.18(5)J.RTF 9 <br />1-07.18(5)J Pollution Liability 10 <br />(January 4, 2016 APWA GSP) 11 <br /> 12 <br />The Contractor shall provide a Contractors Pollution Liability policy, providing coverage for 13 <br />claims involving bodily injury, property damage (including loss of use of tangible property that 14 <br />has not been physically injured), cleanup costs, remediation, disposal or other handling of 15 <br />pollutants, including costs and expenses incurred in the investigation, defense, or settlement 16 <br />of claims, arising out of any one or more of the following: 17 <br />1. Contractor’s operations related to this project. 18 <br />2. Remediation, abatement, repair, maintenance or other work with lead-based paint or 19 <br />materials containing asbestos. 20 <br />3. Transportation of hazardous materials away from any site related to this project. 21 <br /> 22 <br />All entities listed under 1-07.18(2) of these Special Provisions shall be named by endorsement 23 <br />as additional insureds on the Contractors Pollution Liability insurance policy. 24 <br /> 25 <br />Such Pollution Liability policy shall provide the following minimum limits: 26 <br />*** $2,000,000 *** each loss and annual aggregate 27 <br /> 28 <br />1-07.23.GR1 29 <br />1-07.23 Public Convenience and Safety 30 <br /> 31 <br />1-07.23(1).GR1 32 <br />Construction Under Traffic 33 <br /> 34 <br />1-07.23(1).INST1.GR1 35 <br />Section 1-07.23(1) is supplemented with the following: 36 <br /> 37 <br />COE 1-07.23(1).OPT5.FR1.rtf 38 <br />(October 3, 2022) 39 <br /> 40 <br />*** 41 <br />Lane, ramp, shoulder, and roadway closures are subject to the following restrictions: 42 <br /> 43 <br />No lane closures are allowed on arterials from 2:30 to 5:30 pm. Additional restrictions 44 <br />are listed below. 45 <br /> 46 <br />· 41st St at 3rd Ave SE intersection: No lane closures are allowed between 2:30 47 <br />pm and 6:00 pm. 48 <br />· 41st St at Colby Ave and Hoyt Ave intersections: No lane closures are allowed 49 <br />between 11:00 am and 8:00 pm. 50 <br />· 3rd Ave SE: No single lane alternating two-way traffic under flagger control is 51 <br />allowed between 2:30 pm and 5:30 pm 52
