<br />Lexington-Broadway Stormwater Imp. Phase 1 4 Scope of Services <br />C. The standing PM meeting agenda will include a project status update; review of <br />upcoming activities; and discussion of scope, schedule, and budget. The Consultant will <br />email draft meeting summary notes within 7 business days following each meeting and <br />will incorporate City comments, as appropriate, in the final notes. <br />3. A total of three project schedule updates are assumed. <br />4. Invoices will be provided in the Consultant’s standard invoice format. <br />5. Expense backup will not be provided with invoices but will be available for review upon <br />request. <br />Deliverables <br />The following deliverables will be produced: <br />1. Monthly status reports, budget updates, and invoices (emailed Portable Document Format <br />[PDF]). <br />2. PM meeting agenda and notes (emailed PDF files). <br />Task 200 Survey and Utility Location <br />Objective <br />The task objective is to obtain utility pothole locates and survey of the locations in support of <br />project design. David Evans and Associates (DEA) is being retained to provide these services. <br />Services under this task are contingent in nature. The specific scope of services, estimated <br />labor, and associated expenses are to be negotiated in advance and agreed upon by both the <br />City and the Consultant prior to NTP. <br />Consultant Services <br />The Consultant will provide the following services: <br />1. Review existing utility information to determine locations where additional pothole and <br />survey information may be needed to support project design. <br />2. Perform underground utility locates. <br />3. Provide utility potholing that meets Subsurface Utility Engineering (SUE) Quality Level A for <br />utility locations according to the American Society of Civil Engineers (ASCE) standard <br />guidelines regarding utility location, depth, and size information to support project design. <br />4. Complete a survey of the pothole locations and provide record of utility locate information, <br />including location, depth, width (for duct banks), dimensions, and material. <br />5. Integrate the locates into the project Civil 3D maps originally developed under the <br />Lexington-Broadway Preliminary Design survey. <br />6. Provide supplemental survey, upon written request, to support project design.
