Edgewater Creek Bridge Replacement November 2023 7 Each Proposal shall be submitted in a sealed envelope, with the Project Name and 1 Project Number as stated in the Call for Bids clearly marked on the outside of the 2 envelope, or as otherwise required in the Bid Documents, to ensure proper handling and 3 delivery. 4 5 To be considered responsive on a FHWA-funded project, the Bidder may be required to 6 submit the following items, as required by Section 1-02.6: 7 8 • DBE Utilization Certification (WSDOT 272-056) 9 • DBE Written Confirmation Document (WSDOT 422-031) from each DBE firm 10 listed on the Bidder’s completed DBE Utilization Certification 11 • Good Faith Effort (GFE) Documentation 12 • DBE Bid Item Breakdown (WSDOT 272-054) 13 • DBE Trucking Credit Form (WSDOT 272-058) 14 15 DBE Utilization Certification 16 The DBE Utilization Certification shall be received at the same location and no later than 17 the time required for delivery of the Proposal. The Contracting Agency will not open or 18 consider any Proposal when the DBE Utilization Certification is received after the time 19 specified for receipt of Proposals or received in a location other than that specified for 20 receipt of Proposals. The DBE Utilization Certification may be submitted in the same 21 envelope as the Bid deposit. 22 23 DBE Written Confirmation and/or GFE Documentation 24 The DBE Written Confirmation Documents and/or GFE Documents are not required to 25 be submitted with the Proposal. The DBE Written Confirmation Document(s) and/or GFE 26 (if any) shall be received either with the Bid Proposal or as a Supplement to the Bid. The 27 documents shall be received no later than 48 hours (not including Saturdays, Sundays 28 and Holidays) after the time for delivery of the Proposal. To be considered responsive, 29 Bidders shall submit Written Confirmation Documentation from each DBE firm listed on 30 the Bidder’s completed DBE Utilization Certification and/or the GFE as required by 31 Section 1-02.6. 32 33 DBE Bid Item Breakdown and DBE Trucking Credit Form 34 The DBE Bid Item Breakdown and the DBE Trucking Credit Forms (if applicable) shall be 35 received either with the Bid Proposal or as a Supplement to the Bid. The documents 36 shall be received no later than 48 hours (not including Saturdays, Sundays and Holidays) 37 after the time for delivery of the Proposal. To be considered responsive, Bidders shall 38 submit a completed DBE Bid Item Breakdown and a DBE Trucking Credit Form for each 39 DBE Trucking firm listed on the DBE Utilization Certification, however, minor errors and 40 corrections to DBE Bid Item Breakdown or DBE Trucking Credit Forms will be returned 41 for correction for a period up to five calendar days (not including Saturdays, Sundays 42 and Holidays) after the time for delivery of the Proposal. A DBE Bid Item Breakdown or 43 DBE Trucking Credit Forms that are still incorrect after the correction period will be 44 determined to be non-responsive. 45 46 Supplemental bid information submitted after the proposal submittal but within 48 hours 47 of the time and date the proposal is due, shall be submitted as follows: 48 49 1. In a sealed envelope labeled the same as for the Proposal, with “Supplemental 50 Information” added, or 51 2. By facsimile to the following FAX number: N/A, or 52
