8 <br /> <br />SECTION THREE <br />SUBMITTAL REQUIREMENTS AND EVALUATION <br />3.1 SUBMITTAL INFORMATION <br />The statements of qualifications must be limited to fifteen (15) pages, which does not include the grant <br />clauses or required forms. Single-sided printing is not required. The front and back cover are not <br />considered a page. No additional information or pamphlets will be considered by the selection <br />committee. Contact information, including contact person, e-mail, and phone number, should be <br />provided on the first page of the submittal. Include the executed Community Development Block Grant <br />clauses and federal forms with the submission. <br />Title each page with your name or company name and note the section number for each response. <br />Statements of Qualifications must include the Signed Authorization contained on the last page of this <br />RFQ, or they may be deemed non-responsive and may not be evaluated. <br />3.2 SUBMITTAL FORMAT <br />Statements of Qualification will be evaluated on the completeness of the information supplied in the <br />submittal. <br />Submit Statements of Qualification with the following three-section format. Failure to clearly and <br />completely provide all the information below may result in rejection as non-responsive. <br />1. QUALIFICATIONS & EXPERIENCE <br />Provide a general overview of your organization. At a minimum, answer the following <br />questions: <br />a) Describe your company. Include how long the company has been in business. Describe <br />the qualifications of your company, its business experience, and achievements. <br />b) Who are you proposing to be the project manager if awarded this contract? What is their <br />experience with this work and other aspects pertinent to this project? What are their <br />years of experience, years in the profession, years with the firm, years of applicable <br />licenses, etc.? Provide a list of three major projects that person has been involved in and <br />their role. Full resumes are discouraged from being included. <br />c) Provide names, tenure, roles, and responsibilities for each key team member engaged in <br />providing the related services. <br />d) Describe relevant team member experience with permitting, designing, and <br />constructing federally funded projects. <br />2. APPROACH & CAPABILITY <br />a) Where is the staff associated with this project located, and what is the firm’s availability <br />to complete the work within the stated time period?
