Asbestos and Lead Inspection <br />Everett Parks and Facilities Department | Everett, Washington <br />July 2, 2024 | Terracon Proposal No. P81247400 <br />Facilities | Environmental | Geotechnical | Materials 1 <br /> <br /> <br /> <br />DETAILED SCOPE OF SERVICES <br />1.0 PROJECT INFORMATION <br />We understand that the Client is requesting an inspection for asbestos-containing materials <br />and lead-containing paints prior to future renovation. The inspection is limited to accessible <br />areas of the interior, exterior, and roofs of the buildings at the above referenced addresses <br />located in Everett, Washington (see attached Project Area Figure). The inspection does not <br />include equipment or supplies in the building that are not part of the building systems. <br />Terracon also understands that the intent of the inspection is to assist the Client with <br />communicating the presence of lead-containing paint and the presence, location, and <br />quantity of asbestos-containing material (ACM) to employees, vendors, and contractors <br />working in the buildings and to meet the requirements for an asbestos survey for the Puget <br />Sound Clean Air Agency (PSCAA) and a good faith inspection as required by Washington <br />State Department of Labor and Industries’ Division of Occupational Safety and Health <br />(DOSH) regulations prior to renovation. <br />This scope of work was developed with the assistance of the following documents and <br />individuals: <br /> Ms. Lolly Huggins, Capital Projects Coordinator, Everett Parks and Facilities <br />Department <br /> Forest Park As Built, Sheet 1, 12.16.2003 <br /> <br /> Legion Park Foundation Plan, Sheet 1, 1.24.1995 <br /> <br /> Thornton Sullivan Restroom Roof Replacement Draft, Sheet 1, 7.13.2004 <br /> <br />2.0 SCOPE OF SERVICES <br />2.1 Asbestos Inspection <br />Pursuant to the Client’s request, Terracon will perform an asbestos inspection of the three <br />restroom facilities identified herein. Terracon anticipates to collecting up to 75 samples for <br />this project. If additional samples are collected, Terracon will contact the Client for <br />authorization prior to incurring additional project costs. If authorized, additional samples will <br />be billed at $25.00 per sample. <br />The samples will be submitted and analyzed for asbestos content by polarized light <br />microscopy (PLM) by a laboratory accredited by the National Voluntary Laboratory <br />Accreditation Program (NVLAP) on a 5-Day turnaround time (TAT). Terracon may also <br />collect quality control asbestos bulk samples that will be submitted to an independent <br />laboratory for analysis.
