<br />Exhibit A (Scope of Work) <br />SCOPE OF WORK <br /> <br />Bridgeways/Everett SUD Court Program <br /> <br />I. General <br /> <br />Service Provider agrees that the purpose of all Work done in furtherance of this <br />Professional Services Agreement is to identify individuals before the Everett <br />Municipal Court with substance use disorders or other behavioral health needs <br />and engage those individuals with rapid assessment tools and community- <br />based therapeutic interventions. <br /> <br />II. Program Staffing <br /> <br />SUD Professional (SUDP) is responsible for: <br /> <br />• Provide access to appropriate assessments for SUD to defendants at the <br />Everett Municipal Court in a timely manner (likely to be the date of <br />arraignment or as soon thereafter as possible). <br />• Document assessment results based on the ASAD medicine placement <br />criteria. <br />• Following the assessment, if enrollment into a drug treatment program is <br />recommended, determine the proper course of care (service plan) <br />including recommendations for treatment options in the shortest amount <br />of time possible. May follow up on treatment recommendations and <br />facilitate admissions to treatment facilities as appropriate. <br />• Utilize case management services and one-to-one sessions to <br />manage case to successful outcomes, including sustainable stability <br />and community responsibility; develop goals, promote personal <br />responsibility for recovery. <br />• Jointly serve with the TCCM as the liaison to program. <br /> <br />Therapeutic Court Case Manager/Liaison (TCCM) is responsible for: <br /> <br />• Jointly serve with the SUDP as the liaison to program. <br />• Supporting the SUDP/TCPM in accordance with established program <br />guidelines by coordinating covered recovery supports services that are <br />necessary for long-term support and services. This may include <br />procurement of bus cards or other transportation services, clothing, basic <br />needs, utility and/or phone assistance, and routine drug testing. <br />• Providing support to SUDP during court sessions which may include <br />coordinating schedules, working with clients to assist in scheduling required <br />appointments and general client communications. <br />• Utilize case management services and one-to-one sessions to
