Jobs are not scheduled until signed <br /> paperwork is received by Davison Roofing. <br /> Homeowner Signature: Date: <br /> Submitted B Date: <br /> Prices are guaranteed if signed within 15 days. <br /> Davison Roofing Customer Care Program <br /> To protect you and your home we will: <br /> ✓ Make a plan to address any special concerns you have. <br /> ✓ Keep the work site clean and organized daily. <br /> ✓ Follow safety regulations. <br /> ✓ Magnet the premises for nails daily. <br /> ✓ Create a specific safety plan for your job site. <br /> ✓ Keep you informed through the entire roofing process. <br /> ✓ Ask you about delicate landscape items and place plywood/tarps to protect them. <br /> To ensure a problem free roof installation,we suggest that you: <br /> ✓ Consider putting fragile items in a secure location that may fall during material loading/the roofing process. <br /> ✓ If we are installing or replacing plywood,cover or remove items in the attic that may get dirty or damaged. <br /> ✓ If you have skylights,they will be removed to install new flashing.Debris such as pine needles may fall <br /> through the skylight well during skylight removal.You may wish to lay something down to protect the floor. <br /> ✓ Be prepared for noise; if you have sensitive pets you may want to consider boarding. <br /> ✓ If you have a roof mounted dish/antenna it will need to be removed during installation and should not be re- <br /> installed on the roof. To avoid service disruptions,you may wish to contact your service provider to have the <br /> dish/antenna moved off the roof onto a siding mounted bracket system prior to your roofing start date. <br /> Before we come to install your roof,we will: <br /> ✓ Have a Honey Bucket delivered;none of our employees will come inside your home. <br /> ✓ Let you know when we will be there. <br /> ✓ Make sure your roofing product is in stock and ready for delivery. <br /> ✓ Ask you to move your vehicles out of your driveway/garage for the container and material deliveries. <br /> ✓ Ask you to show us what outside power source you would like us to use for our power tools. <br /> ✓ Ask you to please have a deposit check ready on your start date. <br /> After we install your roof,we will: <br /> ✓ Do a walk through with you if requested and address any punch list items. <br /> ✓ Provide a free inspection of your roof if requested,one(1)year after completion. <br /> Once your final invoice is paid in full,we will: <br /> ✓ Register your Certainteed 5-star warranty. <br /> ✓ Send you a receipt and Davison Roofing labor warranty. <br /> Initial <br /> Page 2 of 4 <br /> Category 2:For official use only/disclosure permissible by law. <br />
