<br />• APPLICATION OF AGRICULTURAL CHEMICALS, INCLUDING
<br />FERTILIZERS AND PESTICIDES, SHALL BE CONDUCTED IN A
<br />MANNER AND AT APPLICATION RATES THAT WILL NOT RESULT IN
<br />LOSS OF CHEMICAL TO STORMWATER RUNOFF. MANUFACTURERS'
<br />RECOMMENDATIONS FOR APPLICATION PROCEDURES AND RATES
<br />SHALL BE FOLLOWED.
<br />DEMOLITION:
<br />• DUST RELEASED FROM DEMOLISHED SIDEWALKS SHALL BE
<br />CONTROLLED USING DUST CONTROL MEASURES (BMP C140).
<br />• STORM DRAIN INLETS VULNERABLE TO STORMWATER DISCHARGE
<br />CARRYING DUST, SOIL, OR DEBRIS WILL BE PROTECTED USING
<br />STORM DRAIN INLET PROTECTION (BMP C220 AS INDICATED ON
<br />THE PLAN).
<br />• PROCESS WATER AND SLURRY RESULTING FROM SAWCUTTING
<br />AND SURFACING OPERATIONS WILL BE PREVENTED FROM
<br />ENTERING THE WATERS OF THE STATE BY IMPLEMENTING

<br />CONCRETE AND GROUT:
<br />• PROCESS WATER AND SLURRY RESULTING FROM CONCRETE
<br />WORK WILL BE PREVENTED FROM ENTERING THE WATERS OF THE
<br />STATE BY IMPLEMENTING CONCRETE HANDLING MEASURES (BMP
<br />C151).
<br />1. CONCRETE TRUCK CHUTES PUMPS AND INTERNALS SHALL
<br />BE WASHED OUT ONLY INTO FORMED AREAS AWAITING
<br />INSTALLATION OF CONCRETE OR ASPHALT.
<br />2. UNUSED CONCRETE REMAINING IN THE TRUCK AND PUMP
<br />SHALL BE RETURNED TO THE ORIGINATING BATCH PLANT
<br />FOR RECYCLING.
<br />3. HAND TOOLS INCLUDING, BUT NOT LIMITED TO, SCREEDS,
<br />SHOVELS, RAKES, FLOATS, AND TROWELS SHALL BE
<br />WASHED OFF ONLY INTO FORMED AREAS AWAITING
<br />INSTALLATION OF CONCRETE OR ASPHALT.
<br />WHEN NO FORMED AREAS ARE AVAILABLE, WASHWATER AND
<br />LEFTOVER PRODUCT SHALL BE CONTAINED IN A LINED
<br />CONTAINER. CONTAINED CONCRETE SHALL BE DISPOSED OF IN A
<br />MANNER THAT DOES NOT VIOLATE GROUNDWATER OR SURFACE
<br />WATER QUALITY STANDARDS.
<br />SANITARY WASTEWATER:
<br />• PORTABLE SANITATION FACILITIES WILL BE FIRMLY SECURED,
<br />REGULARLY MAINTAINED, AND EMPTIED WHEN NECESSARY.
<br />• SOLID WASTE WILL BE STORED IN SECURE, CLEARLY MARKED
<br />CONTAINERS.
<br />OTHER:
<br />• OTHER BMPS WILL BE ADMINISTERED AS NECESSARY TO
<br />ADDRESS ANY ADDITIONAL POLLUTANT SOURCES ON SITE.
<br />FIELD MARKINGS
<br />BEFORE ANY CLEARING OR GRADING OCCURS, CONTRACTOR OR
<br />SURVEYOR SHALL INSTALL TEMPORARY FIELD MARKINGS AS FOLLOWS:
<br />1. MARK CLEARING LIMITS AS SHOWN ON PLAN OR AS DIRECTED BY
<br />OWNER (LIMITS ON PLAN ARE MAXIMUM ALLOWABLE) WITH
<br />CONTINUOUS RUN OF ORANGE SURVEYOR'S TAPE 4-6 FEET ABOVE
<br />GROUND. CONTRACTOR MAY SUBSTITUTE CONSTRUCTION OR SILT
<br />FENCING FOR SURVEYOR'S TAPE.
<br />2. CONTRACTOR SHALL REQUEST INSPECTION OF FIELD MARKINGS BY
<br />C.O.E. STAFF PRIOR TO COMMENCING CLEARING OR GRADING.
<br />SOIL STOCKPILES
<br />TEMPORARY STOCKPILE LOCATIONS MAY BE ADJUSTED BY CONTRACTOR
<br />IN FIELD. TOPSOIL STRIPPING STOCKPILES SHALL BE WINDROWED
<br />TEMPORARILY ALONG LOT FRONTAGE FOR SPREADING WHEN LOTS ARE
<br />CLEARED. CARE SHOULD BE TAKEN TO AVOID UNINTENTIONAL DRAINAGE
<br />IMPOUNDMENT OR DIVERSION. TOPSOIL STRIPPINGS MAY BE
<br />INCORPORATED INTO EROSION CONTROL DIVERSION BERMS.
<br />STOCKPILES SHALL NOT BE PLACED WITHIN NGPAs, SCREENING BUFFERS,
<br />EXISTING DRAINAGE OR UTILITY EASEMENTS OR RIGHTS -OF -WAY.
<br />STOCKPILE MANAGEMENT SHALL COMPLY WITH T.E.S.C. GENERAL NOTES
<br />ON SHEET C2.1. INCLUDING STABILIZATION WITHIN 24 HOURS.
<br />CITY OF EVERETT GENERAL NOTES
<br />1. APPROVAL OF THIS EROSION/SEDIMENTATION CONTROL (ESC) PLAN
<br />DOES NOT CONSTITUTE AN APPROVAL OF PERMANENT ROAD OR
<br />DRAINAGE DESIGN (E.G. SIZE AND LOCATION OF ROADS, PIPES,
<br />RESTRICTORS, CHANNELS, RETENTION FACILITIES, UTILITIES).
<br />2. THE IMPLEMENTATION OF THESE ESC PLANS AND THE CONSTRUCTION,
<br />MAINTENANCE, REPLACEMENT, AND UPGRADING OF THESE ESC
<br />FACILITIES IS THE RESPONSIBILITY OF THE APPLICANT/CONTRACTOR
<br />UNTIL ALL CONSTRUCTION IS COMPLETED AND APPROVED AND
<br />VEGETATION/LANDSCAPING IS ESTABLISHED.
<br />3. THE BOUNDARIES OF THE CLEARING LIMITS SHOWN ON THIS PLAN SHALL
<br />BE CLEARLY FLAGGED IN THE FIELD PRIOR TO CONSTRUCTION. DURING
<br />THE CONSTRUCTION PERIOD, NO DISTURBANCE BEYOND THE FLAGGED
<br />CLEARING LIMITS SHALL BE PERMITTED. THE FLAGGING SHALL BE
<br />MAINTAINED BY THE APPLICANT/CONTRACTOR FOR THE DURATION OF
<br />CONSTRUCTION.
<br />4. THE ESC FACILITIES SHOWN ON THIS PLAN MUST BE CONSTRUCTED IN
<br />CONJUNCTION WITH ALL CLEARING AND GRADING ACTIVITIES, AND IN
<br />SUCH A MANNER AS TO ENSURE THAT SEDIMENT AND SEDIMENT LADEN
<br />WATER DO NOT ENTER THE DRAINAGE SYSTEM, ROADWAYS, OR VIOLATE
<br />APPLICABLE WATER QUALITY STANDARDS.
<br />5. THE ESC FACILITIES SHOWN ON THIS PLAN ARE THE MINIMUM
<br />REQUIREMENTS FOR ANTICIPATED SITE CONDITIONS. DURING THE
<br />CONSTRUCTION PERIOD, THESE ESC FACILITIES SHALL BE UPGRADED AS
<br />NEEDED FOR UNEXPECTED STORM EVENTS AND TO ENSURE THAT
<br />SEDIMENT AND SEDIMENT -LADEN WATER DO NOT LEAVE THE SITE. 2022

<br />6. THE ESC FACILITIES SHALL BE INSPECTED ROUTINELY AND MAINTAINED
<br />BY THE APPLICANT/CONTRACTOR TO ENSURE THEIR CONTINUED
<br />FUNCTIONING, ESPECIALLY AFTER STORM EVENTS.
<br />7. THE ESC FACILITIES ON INACTIVE SITES SHALL BE INSPECTED AND
<br />MAINTAINED A MINIMUM OF ONCE A MONTH OR WITHIN THE 48 HOURS
<br />FOLLOWING A MAJOR STORM EVENT.
<br />8. AT NO TIME SHALL MORE THAN ONE FOOT OF SEDIMENT BE ALLOWED TO
<br />ACCUMULATE WITHIN A TRAPPED CATCH BASIN. ALL CATCH BASINS AND
<br />CONVEYANCE LINES SHALL BE CLEANED PRIOR TO PAVING. THE
<br />CLEANING OPERATION SHALL NOT FLUSH SEDIMENT LADEN WATER INTO
<br />THE DOWNSTREAM SYSTEM.
<br />9. STABILIZED CONSTRUCTION ENTRANCES SHALL BE INSTALLED AT THE
<br />BEGINNING OF CONSTRUCTION AND MAINTAINED FOR THE DURATION OF
<br />THE PROJECT. ADDITIONAL MEASURES MAY BE REQUIRED TO ENSURE
<br />THAT ALL PAVED AREAS ARE KEPT CLEAN FOR THE DURATION OF THE
<br />PROJECT.
<br />10. THE CONTRACTOR SHALL PROVIDE PERIODIC STREET CLEANING TO
<br />REMOVE DEBRIS AND SEDIMENT TRACKED OFF THE SITE.
<br />11. APPROPRIATE MEASURES SHALL BE TAKEN TO STOP SEDIMENT FROM
<br />ENTERING SURFACE WATER BODIES IF THE PROPOSED BMPS FAIL. IF
<br />BMPS FAIL AND SEDIMENT -LADEN WATER IS LEAVING THE SITE,
<br />IMMEDIATELY NOTIFY YOUR PUBLIC WORKS INSPECTOR.
<br />12. BARE AND/OR DISTURBED SOILS SHALL REMAIN UNCOVERED AND/OR
<br />UNSTABILIZED FOR NO MORE THAN 2 DAYS FROM OCTOBER 1 THROUGH
<br />APRIL 30, AND FOR NO MORE THAN 7 DAYS FROM MAY 1 THROUGH
<br />SEPTEMBER30.
<br />13. NOTIFY THE CITY PUBLIC WORKS INSPECTOR AT LEAST 24 HOURS PRIOR
<br />TO DEWATERING ACTIVITIES. HANDLE HIGHLY TURBID OR OTHERWISE
<br />CONTAMINATED DEWATERING WATER SEPARATELY FROM STORMWATER.
<br />14. THE APPROVED PROJECT PUBLIC WORKS PERMIT AND SWPPP SHALL BE
<br />RETAINEDON SITE OR READILY AVAILABLE TO THE CONTRACTOR
<br />THROUGHOUT THE DURATION OF THE CONSTRUCTION PROJECT.
<br />15. DESIGN, INSTALL, IMPLEMENT, AND MAINTAIN EFFECTIVE POLLUTION
<br />PREVENTION MEASURES TO MINIMIZE THE DISCHARGE OF POLLUTANTS.
<br />16. PROTECT ALL LID BMPS FROM SEDIMENTATION THROUGH INSTALLATION
<br />AND MAINTENANCE OF EROSION AND SEDIMENT CONTROL BMPS ON
