By e-mail to the following e-mail address: sbridge@everettwa.gov 10 <br /> 11 <br />DBE Utilization Certification (WSDOT Form 272 -056) 12 <br />The DBE Utilization Certification shall be received at the same location and no later than 13 <br />the time required for delivery of the Proposal. The Contracting Agency will not open or 14 <br />consider any Proposal when the DBE Utilization Certification is received after the time 15 <br />specified for receipt of Proposals or received in a location other than that specified for 16 <br />receipt of Proposals. The DBE Utilization Certification may be submitted in the same 17 <br />envelope as the Bid deposit. 18 <br /> 19 <br />DBE Written Confirmation Document (WSDOT Form 422-031) and/or GFE 20 <br />Documentation, (if applicable) 21 <br />The DBE Written Confirmation Documents and/or GFE Documentation are not required to 22 <br />be submitted with the Proposal. The DBE Written Confirmation Document(s) and/or GFE 23 <br />Documentation(if applicable) shall be received either with the Bid Proposal or as a 24 <br />Supplement to the Bid. The Written Confirmation and/or GFE Documentation shall be 25 <br />received no later than 48 hours (not including Saturdays, Sundays and Holidays) after the 26 <br />time for delivery of the Proposal. To be considered responsive, Bidders shall submit a 27 <br />Written Confirmation Document from each DBE firm listed on the Bidder’s completed DBE 28 <br />Utilization Certification and/or the GFE Documentation as required by Section 1 -02.6. 29 <br /> 30 <br />DBE Bid Item Breakdown Form (WSDOT Form 272-054) 31 <br />The DBE Bid Item Breakdown shall be received either with the Bid Proposal or as a 32 <br />Supplement to the Bid. The documents shall be received no later than 48 hours (not 33 <br />including Saturdays, Sundays and Holidays) after the time for delivery of the Proposal. To 34 <br />be considered responsive, Bidders shall submit a completed DBE Bid Item Breakdown, 35 <br />however, the Contractor may correct minor errors to the DBE Bid Item Breakdown for a 36 <br />period up to five calendar days after bid opening (not including Saturdays, Sundays and 37 <br />Holidays). DBE Bid Item Breakdowns that are still incorrect after the correction period will 38 <br />be determined to be non-responsive. 39 <br /> 40 <br />The DBE Bid Item Breakdown will not be included as part of the executed Contract. 41 <br />1-02.13.RTF 42 <br />1-02.13 Irregular Proposals 43 <br />(January 4, 2024 APWA GSP) 44 <br /> 45 <br />Delete this section and replace it with the following: 46 <br /> 47 <br />1. A Proposal will be considered irregular and will be rejected if: 48 <br />a. The Bidder is not prequalified when so required; 49 <br />b.
