CITY OF EVERETT <br /> EVERETT Permit Services <br /> WASHINGTON <br /> 12/28/2021 <br /> Ziply Fiber <br /> Attn: Justin Fontes <br /> 1800 41st St M/C: 20S <br /> Everett, WA 98203 <br /> Subject: 2022 Annual Blanket Utility Permit for Activity in City of Everett Right-of-Way <br /> Blanket Permit Number: BLP2201-004 <br /> Dear Justin Fontes, <br /> The City of Everett has updated the Annual Blanket Utility Permit process and requirements.An Annual Blanket <br /> Utility Permit is required for all Utility Companies prior to the commencement of any work in the public right-of- <br /> way.Additional permits may be required depending on the scope of work proposed, as outlined in Exhibit A <br /> (attached).The attached documents are associated with the 2022 calendar year. <br /> Enclosed please find the following documents: <br /> • 2022 Annual Blanket Utility Permit with Contractor/Subcontractor List and Exhibit A <br /> • City's Assurance Device form (Performance Bond) <br /> • How to Request an Inspection Handout <br /> Please carefully read the enclosed materials and return to the City of Everett's Public Works office (via email or <br /> mail)the following items by February 15`h, 2022: <br /> • Signed complete 2022 Annual Blanket Utility Permit document <br /> • Payment to the City of Everett in the amount of$500.00 for processing of the 2022 Blanket Permit(can <br /> be processes online at https://pw.everettwa.gov) <br /> • Assurance Device with City's Performance Bond form in the amount of$50,000.00 <br /> • Completed list of Contractors/Subcontractors that may be performing work for your company. <br /> Once all materials are received, signed, and processed,your 2022 Annual Blanket Utility Permit will become <br /> active.An approved, signed copy of the 2022 Annual Blanket Utility Permit will be uploaded to eTRAKiT for your <br /> records. Please note that a copy of the 2022 Annual Blanket Utility Permit and Exhibit A shall be forwarded to all <br /> contra ctors/subcontractors/permittees that will be doing work for your company, so that each party involved in <br /> the proposed work is aware of all requirements and able to comply with this permit.
