CITY OF EVERETT <br /> EVERETT Permit Services <br /> WASHINGTON <br /> After signing, please return to: <br /> Permit Services Manager <br /> City of Everett Public Works Department <br /> 3200 Cedar Street <br /> Everett,WA 98201 <br /> Date: 12/28/2021 Blanket Permit <br /> #: BLP2201-004 <br /> 2022 CITY OF EVERETT PUBLIC WORKS <br /> ANNUAL BLANKET UTILITY PERMIT <br /> FOR FRANCHISE UTILITY WORK WITHIN THE PUBLIC RIGHT-OF-WAY <br /> The City of Everett hereby grants an Annual Blanket Utility Permit to Ziply Fiber,whose address is 1800 41st St M/C: <br /> 20S, Everett, WA, 98203 hereinafter referred to as "Permittee," for a term commencing on the 1st day of January, <br /> 2022, and terminating on the 31st day of January, 2023, unless otherwise notified. <br /> This Permit is subject to the following conditions: <br /> 1. This permit authorizes the Permittee the opportunity to perform activities in the public right-of-way, as <br /> described in Exhibit A, which is attached hereto and incorporated herein by reference. Each categorized <br /> activity will require specific action items to be taken prior to the commencement of any work in the public <br /> right-of-way. <br /> 2. The Permittee must pay the City an Annual Blanket Utility Permit processing fee in the amount of five- <br /> hundred dollars($500.00) to be paid at the time this permit is signed and returned to the City for processing <br /> and issuance of this Annual Blanket Utility Permit. Payment can be made online using your company account <br /> on the eTRAKiT Online Permitting Portal at https://pw.everettwa.gov. In order to pay from your dashboard, <br /> you may need to link to this permit number if it is not already shown. Your account must match the <br /> information on file in order to link. If the City needs to update company contact information for processing <br /> Annual Blanket Utility Permits, please contact Sabrina Good at SGood@everettwa.gov. <br /> 3. The Permittee shall provide the City with an Assurance in the amount of fifty-thousand dollars ($50,000.00) <br /> to ensure the faithful performance of its obligations to restore City streets and other property, pursuant to <br /> EMC 13.32.010 and this permit. Permittee shall keep the Assurance or Bond in full force and effect until all <br /> construction begun under this permit has been completed and all restoration of public and private property <br /> has been completed. The Assurance shall be in the form of a Performance Bond, and shall be provided on <br /> the Performance Bond document provided by the City. <br /> 4. For each separate use of the right-of-way under this Annual Blanket Utility Permit and prior to commencing <br /> any work in the right-of-way under this Permit, the Permittee shall follow the required actions outlined <br /> below as categorized by Activity Type in Exhibit A.All permits must be applied for through the City's eTRAKiT <br /> online permitting portal (https://pw.everettwa.gov) and plan sets and traffic control plans shall be uploaded <br /> when applying for the permit. Emailed or mailed packages will not be accepted and will be considered void, <br /> unless otherwise agreed upon with the City reviewers on a project-specific basis. <br /> a. For Minor Activities: as defined in Exhibit A, no further actions are required prior to commencing <br /> work in the right-of-way as long as a valid Annual Blanket Utility Permit has been issued for the year <br /> in which the activity will take place. Minor activities are limited to work that does not require traffic <br /> control for temporary closure of lanes of travel or sidewalks, does not involve pavement restoration <br /> Pagel of 8 <br />
