CITY OF EVERETT <br /> EVERETT Permit Services <br /> WASHINGTON <br /> done without the permit on-site is subject to a Stop Work Order by the Public Works <br /> Inspectors. By purchasing the permit and commencing work, the Permittee assumes all <br /> responsibility of and agrees to all conditions printed on the face of the permit and/or as <br /> marked up on the approved set of plans. As a result, a signature by the Permittee will not <br /> be required at permit issuance. <br /> vi. Inspection Scheduling: Inspections and permit finals are required for all Franchise Utility <br /> Permits (PW). Inspection requests must be formally made through the eTRAKiT Online <br /> Permitting Portal (https://pw.everettwa.gov) and are required at the following stages of <br /> construction: 1) job start notification, 2) potholing for COE utility crossings, 3) Utility <br /> work/cold patch complete,4) Pavement subgrade evaluation, 5) Permanent pavement final, <br /> and 6) Permit Final. Inspections can be requested up to seven days in advance and the cut <br /> off time is 6am the day of the inspection. If inspections need to be cancelled, they shall be <br /> cancelled by 11pm the night prior to the scheduled inspection at the latest. <br /> • The Permittee is required to schedule a Job Start Notification prior to <br /> commencing work. This is the first inspection required for the job and shall be <br /> requested through the eTRAKiT permitting portal. After this, other inspections shall <br /> be scheduled based on the nature of the job and the Public Works Inspector's <br /> instructions during the on-site consultation. <br /> • When applicable, an inspection shall be requested for the inspector to <br /> come out while the contractor is potholing so that the inspector can have <br /> eyes on the COE Utilities and the installed crossings. <br /> • When applicable, an inspection shall be requested for the inspector to see <br /> the utility work completed, in the process of backfill, and/or while <br /> completing the cold patch. <br /> • Per COE Standard Drawing#326, final restoration is required to take place within <br /> 2 weeks of completion of work within the roadway area (cold patch), unless <br /> otherwise stated on the Franchise Utility Permit. The inspection request for <br /> permanent pavement subgrade shall be made as soon as the restoration date has <br /> been scheduled. <br /> • A Permanent Pavement Final inspection shall be requested for the inspector to <br /> verify that the work has been completed and has been installed to City Standards. <br /> A photo of the completed pavement shall be uploaded to the permit through the <br /> eTRAKiT permitting portal. The inspector has the option to either approve the <br /> inspection from the photo or make a site visit. <br /> • All jobs must request a Public Works Final inspection for the permit to be closed <br /> out and the work done to be considered legal and permitted. <br /> vii. Photo Requirements: Photos shall be provided by the contractor at each of the above listed <br /> inspection stages and uploaded to the eTRAKiT permitting portal, unless otherwise specified <br /> by the City Inspector.To upload a photo, log into the eTRAKiT Permitting Portal, click on the <br /> subject permit, click on the attachments section, and upload the appropriately named and <br /> dated photo. Then from the inspection request page, there is a Notes field for where <br /> reference to an uploaded photo can be made while submitting the inspection request. <br /> Without this note, there will be no way for the City Inspector to know that attachments <br /> were uploaded. If the City Inspector decides that the photo is sufficient for the inspection, <br /> they may choose to result the inspection without a physical site inspection. If the City <br /> Inspector is onsite for an inspection, they may take a photo and upload it rather than <br /> requiring the contractor to do so, however this responsibility falls on the contractor unless <br /> previously communicated otherwise by the City Inspector. <br /> Page 4 of 8 <br />
