This is data only and <br />does not include the Comparative Analysis. <br />$2,000 <br />Section 105(h) Non- <br />Discrimination Testing <br />Completion of annual test to confirm compliance with <br />Section 105(h) requirements. Fees are per calculation.$1,375 <br />ACA Support Services Buy <br />Up Option 1 --1094 & 1095 <br />Filing Service <br />ACA Support Services to assist clients in calculating, defining and <br />reporting the necessary data, including filing the necessary 1094- <br />C and 1095-C forms. The service includes: • Employer Mandate <br />Consulting with an ERISA attorney • 1095-C Benefit Information <br />Reporting • Production & E-filing with the IRS of Form 1094-C • <br />Production & E-filing with the IRS of Form 1095-C (employee form, <br />also called the individual statement) provided to employer in <br />electronic format for distribution to employees • Printing and <br />mailing of Form 1095-C to employees (optional –additional fees <br />apply) <br />$4,000 Per Client + <br />$500 For Each <br />Additional Employer <br />Identification Number <br />(EIN) + $9-$15 (fee <br />varies based on <br />employer size) <br />Statement Fee Per <br />Employee Form <br />including Printing and <br />Mailing. <br />ACA Support Services Buy <br />Up Option 2 -Full Time <br />Employee Tracking & <br />Calculation <br />ACA Support Service to identify full-time employees. This annual <br />service provides reporting for both current employees and new <br />hires to determine if, based on variable hours or fluctuating work <br />schedules, they qualify as full-time under the employer mandate. <br />Annual Tracking -$10 <br />Per Employee Per Year <br />with a Minimum of <br />$2,000 and a <br />Maximum of $7,500 <br />Monthly Tracking - <br />$500 per Month <br />MA-1099 Filing Waived <br />We may receive revenue cost-share from vendor who provide services to the Plan Sponsor through our vendor contract.
