Page 12 of 27 <br />2.10 CITY OF EVERETT RESPONSIBILITIES & PROVIDED SERVICES <br />City of Everett staff will be responsible for and provide the following: <br />• The City of Everett Community Support Manager or designate will oversee all aspects of the <br />contract, including but not limited to monitoring and reviewing the supplier’s performance, <br />reports, and data provided under this contract, approval of submitted invoices, and obtaining <br />approval of all changes in the contract. <br />• The City of Everett Community Support Manager or designate will provide onsite coordination, <br />support in the client referral process, tracking, and assistance with coordinating the day-to-day <br />deployment of contracted case management staff. Clinical supervision will remain the <br />responsibility of the contractor. <br />• Review and approve the qualifications and experience of staff providing service under this <br />contract, including new and replacement staff proposed by the Supplier. The City of Everett also <br />reserves the right to require contractor staff to be replaced because of justifiable complaints by <br />program customers, their legal guardians, support systems, or allied agencies. <br />2.11 QUALITY ASSURANCE <br />The Supplier must ensure staff is eligible to provide case management services for individuals and, at <br />minimum, must provide: <br />1. Education & Experience Verification <br />• The supplier will select case managers for this position based on having experience working with <br />the populations described and who have experience in working with interdisciplinary teams. <br />• Before hiring any employee, the supplier will verify that the employees assigned to the contract <br />have the stated education and experience in managing case management programs, homeless <br />outreach, shared decision-making, collaboration, and managing those with behavioral health <br />concerns. <br />2. Background Check <br />Before hiring any employee, the Supplier must obtain a nationwide criminal background check. <br />Records of criminal background checks must be kept on file by the Supplier and made available to <br />the City of Everett for review upon request. Proposers shall describe their source and method of <br />obtaining background checks as part of their proposal as part of their answer to Form 4.03, 2G. After <br />work begins, any new additional or added employees, volunteers, or interns of the Contractor must <br />also comply with the city of Everett’s security background check requirement as stated above. <br />3. Safety & Emergency <br />The proposer must provide the City of Everett with copies of its agency’s written safety and <br />emergency policies and procedures as part of their proposal response to Form 4.03, 2G. <br />2.12 CONTRACT TRANSITION <br />In the event of a follow-on contract award to another Supplier, the Supplier will ensure a cooperative and <br />smooth transition with a new contract provider or the City of Everett, such as turnover of records, status, <br />reports, etc. <br />2.13 PRICING ADJUSTMENTS <br />Prices must remain firm for the duration of the initial contract period. Reasonable price changes based on <br />market conditions and price or cost analysis may be made after the initial contract period. The supplier <br />must supply documentation that is satisfactory to the City of Everett, such as documented changes to the <br />Consumer Price Index for the Seattle-Tacoma-Bellevue area. <br />Exhibit C
